Provide and Transport radioactive materials to and from Elmendorf AFB Nuclear Medicine Department-This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6. PLEASE READ THIS ENTIRE NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. . This is a Sole Source acquisition to Alaska Radiological Pharmacy LLC. This will be a Blanket Purchase Agreement to provide and transport radioactive materials to and from Elmendorf AFB, Nuclear Medicine Department. This announcement constitutes the only solicitation: a quote is being requested and a written solicitation will not be issued. Solicitation reference number is 3MDG02A0002 is issued as a Request for Quotation (RFQ). The incorporated document provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The NAICS Code is 621511. PROJECT: SCOPE OF WORK. The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items necessary to provide delivery, survey, and pick-up of all radioactive materials related to the preparation of Nuclear Medicine patient care unit doses. 1.1 The primary purpose of this service is to transport radioactive materials to and from =20 the Elmendorf AFB Nuclear Medicine Department. 1.2 The contractor shall act as the shipper and receiver or radioactive material and perform door-to-door delivery and pick up of radiopharmaceuticals. 1.3 The contractor shall hand-carry any radioisotopes in a Type A Nuclear Regulatory =20 Commission (NRC) approved container. 1.4 The contractor shall provide daily service to Elmendorf Nuclear Medicine Department. They shall transport up to three (3) deliveries per day between the hours of 6:00AM and 4:00PM. Any deliveries requested after 4:00PM, may be charged an additional delivery charge per visit. Examinations requested on the weekend shall be prepared and delivered as requested. Each weekend shipment will be assessed a delivery charge annotated in the contractor?s price list. 1.5 The contractor shall maintain compliance with Nuclear Regulatory Commission=20 (NRC) regulations for radioactive material (RAM) permit. 1.6 The contractor shall ensure their radioactive materials license is current and nuclear=20 laboratory is in compliance with set NRC standards. 1.7 The contractor shall order RAM from prime radioisotope manufacturers such as Dupont, Mallinckrodt, and Amersham or other pre-appoved suppliers and receive radiopharmaceutical shipments from the continental United States. 1.8 The contractor shall adequately label all required daily shipment of radioisotopes as well as be responsible for quality control. 1.9 The contractor shall prepare chromatomography and chemical impurity results of radioisotopes at the nuclear laboratory. 1.10 The contractor shall be the supplier of point and flood sources for gamma camera quality control and orientation documentation used in patient procedures. 1.11 The contractor shall use required supplies to complete extraction for white blood cell labeling to rule out acute infection. 1.12 The contractor shall alternate the suppliers of pharmaceuticals for related nuclear medicine procedures and provide alternate dose calibrators for quality control of ordered ?unit doses?. 1.13 The contractor shall furnish all technical equipment to complete dose calibrator quality control and all accessories related to nuclear medicine equipment. 1.14 The contractor shall provide daily register of radioisotope charges; invoices shall be provided to the customer at the end of each week. 2.0 Hours of Operation: The contractor shall prepare and deliver radioisotpes when requested from customers registered with the NRC or Radioisotope Committee (RIC). Office hours are from 6:00AM-4:00PM Monday through Friday; after hours and weekend emergencies will be handled on a case by case basis as needed. The Nuclear Pharmacy will deliver emergent orders within one hour of notification. Daily orders will be delivered during the early morning surge with second deliveries being made about 10:00AM. Should the customer require additional deliveries other than the normal set times of the regular workday, special arrangements can be made with the nuclear pharmacy staff. 3.0 Qualifications/Licenses: The vendor shall provide a current copy of the radioactive materials license endorsed by the NRC. It is the responsibility of the nuclear pharmacy to maintain compliance with the NRC in relations to RAM protocols. Training certificates for staff nuclear pharmacists shall be made available to confirm appropriate credentials. 4.0 Travel Expenses: The contractor shall pay for all travel expenses involved in receiving radiopharmaceuticals at the nuclear pharmacy and/or delivering the radioisotopes to the customer. Delivery terms shall be FOB Destination, Elmendorf AFB, Alaska 99506. The following provisions and clauses are incorporated and shall remain in full force in any resultant purchase order; FAR Part 52.212-1 Instructions to Offerors-Commercial Items, is incorporated by reference. The provision at 52.212-2 Evaluation of Commercial Items, applies to this solicitation. The Government reserves the right to make award on the initial quote received without discussions. Each offeror shall include a completed copy of the provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Items with its quote. FAR Part 52.212-4 Contract Terms and Conditions-Commercial Items, is incorporated by reference and applies to this acquisition. Note the following addenda to FAR 52.212-4: Incorporate DFARS 252.204-7004 Required Central Contractor Registration. Incorporate the clause by reference at 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. However, for paragraph (b) only the following clauses apply: 52.222-21 Prohibition of Segregated Facilities, 52.222-26 Equal Opportunity, 52.222-35 Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era, 52.222-36 Affirmative Action for Handicapped Workers with Disabilities, 52.222-37 Employment Reports on Disabled Veterans and Veterans of the Vietnam Era, and FAR 52.222-48 Exemption from Application of Service Contract Act Provisions for Contracts for Professional Services -- Contractor Certification. FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Order?Commercial Items. DFARS 252.222-7000 Restrictions on Employment of Personnel (Mar 2000)(IAW DFARS 222.7003). This is a DO rated order with priority identification symbol of S10. A Blanket Purchase agreement will be issued for this service. Submit signed and dated offer to 3CONS/LGCB at or 36 December 2001 3:30 PM Alaska Standard Time. Quotation shall be signed, dated and submitted on your company?s letterhead or quotation forms. Quotations may be submitted via fax.
