Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is TIRSE-02-Q-00010 and is being issued as a request for quotations. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. This requirement is set-aside for small businesses for which the Standard Industrial Classification code, 8011. The Internal Revenue Service requires part-time, professional services of a medical doctor, properly licensed and certified in the State of Georgia as a General Practitioner or Internist. Services will be performed at the Internal Revenue Service (IRS), Atlanta Submission Processing Center (ATSPC), Health Unit, 4800 Buford Highway, Chamblee, GA. The initial period of performance will begin January 2002 and go through September 30, 2002, with two one-year option periods not to exceed 33 months. CLIN 0001 Physician Services -- BASIC SERVICES: The contractor shall provide on-site medical services to the IRS nursing staff and, therefore, to IRS employees located at the Center. Services shall be performed on an as needed basis, likely 4 hours one day per month, with the primary tour of duty from 7:30 am -- 11:30 am. The contractor shall be available via pager or cellular phone during clinic hours. The contractor may occasionally be required to schedule hours in the afternoon to perform services for employees working on the night shift. Contract services will consist of the following: 1. Conduct an estimated 30 comprehensive annual physical examinations as scheduled by IRS. For all male employees, rectal-prostate examinations shall be included. 2. In conjunction with conducting physical examinations, the contractor shall review and make recommendations to employees on results from the following tests (which have been performed by the IRS nursing staff or an outside source):(a) Laboratory reports -- blood and urine, (b) Electrocardiogram,(c) Visual acuity,(d) Pulmonary function (e) Hemoccult slides,(f) P.P.D. (tuberculin skin test), (g) Mammogram reports. 3. Provide medical direction for the IRS registered nursing staff. 4. Provide periodic in-service education for the professional nursing staff and employees on current medical issues and other requested topics. 5. Provide medical direction during the Health Unit's fall flu shot promotion. This includes reviewing and updating consent forms, patient education materials and emergency procedures. 6. Review and make appropriate changes to the IRS Health Unit medical procedure book and establish standing orders. 7. When requested by nursing staff, review medical documentation submitted by an employee due to medical absence from work or as a result of illness or injury. 8. At the nurse's discretion, may be requested to assess an employee to determine the urgency of a specific medical complaint, or consult on a job-related accident or emergency. 9. Provide to the nursing staff, on-going guidance and in-service education on occupational exposure to blood-borne pathogens and other OSHA related regulations. 10. Advise management on questions of physical qualifications, health and safety, and other aspects of occupational and preventive medicine. 11. As requested by the nurse(s), provide advice outside tour of duty hours via telephone. A maximum of 10 such "advice calls" per month shall be provided at no additional charge to the Government. This requirement will cover unusual circumstances where a question arises concerning employee treatment for other than an emergency situation. (Calls exceeding 10 per month will be handled under "Additional Services" -- see below.) NOTE: The contractor is not responsible for patient treatment under emergency situations. IRS personnel will refer the patient to the nearest emergency facility or call "911." 12. Provide other related services which may arise during contractor's tour of duty hours. These are sporadic or infrequent requests for advice of a routine nature. CLIN 0001A -- ADDITIONAL SERVICES -- Additional services are defined as being optional services specifically requested by the Government on an "as needed" basis. These services will cover requests for telephone advise in excess of the 10 "advice calls" per month provided under Basic Services (see Item 12 above). Additional services are to be performed, as requested, in addition to and in accordance with the contract terms and conditions for basic services. Requests for Additional Services will be placed verbally by designated personnel, a list of whom will be provided in writing by the IRS. Compensation will be based on the prices for Additional Services as shown in the schedule. CONTRACTOR RESPONSIBILITIES- The contractor is responsible for providing the professional services listed above, and for possessing all licenses, certificates and insurance necessary for performance under this contract. The Government assumes no liability for the acts of the contractor with respect to the diagnosis or treatment of an employee's health problem(s). Services performed by the contractor will not be subject to the supervision or control of any employee of the Government with respect to the method used to perform the services. However, Government reserves the right to request medical opinions of other specialists. GOVERNMENT RESPONSIBILITIES -- The IRS shall provide a fully equipped health unit at the ATSPC where the contractor will render professional services. The IRS Health Unit will have a staff of occupational health nurses. Typing and other clerical services will be provided by the IRS, and an IRS-contracted laboratory will be responsible for the blood and urine tests. EKGs will be provided and paid by the IRS. The contractor will be provided a convenient on-site parking space at no charge. Nothing in this contract, either by intent or implication, is to be construed as establishing an "employer/employee" relationship between the Government and the contractor. WORK SCHEDULE- Within 5 calendar days following contract award, contractor shall submit a work schedule to the IRS for review and approval. In this work schedule, the contractor shall set forth the day during the month which will be spent performing contract services. Any proposed change to the established work schedule must be submitted for approval a minimum of 10 calendar days in advance of such proposed change. The change must be mutually agreed upon between the contractor and the IRS. The contractor will be entitled to take three 4-hour weeks of vacation at his/her own expense. Vacation schedule must be submitted to the IRS for approval a minimum of 30 calendar days in advance. Any unscheduled absence from work will result in a prorated deduction from the price paid to the contractor. The contractor will not be required to work and will not be paid for the 10 Federal holidays. The contractor shall maintain the appropriate licensing and insurance to practice within the state of Georgia. Contractor must provide references of good clinical work and ethical conduct. Contractor shall be required to provide proof of medical liability insurance in accordance with the requirements of Federal Acquisition Regulation (FAR) 52.237-3, Indemnification and Medical Liability Insurance, prior to award of contract. The provisions at FAR 212-1, Instructions to Offerors ?Commercial Items, specifically paragraphs (a), (b)(1), (b)(6), (b)(7) and (b) (11); 52.212-2, Evaluation-Commercial Items, apply to this solicitation.
