COMBINED SYNOPSIS/SOLICITATION WASHERS AND DRYERS: Appliances, removal of old appliances and installation of new appliances. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation F65501-01-R-0034 is issued as a Request for Quotation (RFQ). The NAICS code is 335224. This acquisition is unrestricted. DESCRIPTION: Washers and dryers for base housing residents. This is to include the removal and disposal of old appliances, and installation of new appliances is required (instructions are given below). The appliance specifications are as follows: Washer: Energy Star Compliant, sound deadening, minimum 2.65 Cubic Feet, minimum 5 cycles, minimum 3 water temp., minimum 5 water levels, bleach dispenser, modified energy factor: minimum 1.28, Color: white on white, UL approval. Suitable Appliances: Whirlpool part #GSW9545JQ, Frigidaire part #FWTR425RHS, Whirlpool part #GSW9650KW, or equal. Quantity: 278 each. Dryer: Energy Factor 3.01 or less, sound deadening, minimum 5.5 cubic foot, minimum 4 cycles, minimum 4 heat temp, moisture sensor, top lint filter, color: white on white, UL approval. Suitable Appliances: Whirlpool part #GEW9878JQ, , Frigidaire part #FDE336RE, Whirlpool part #GEW9858JQ, or equal. Quantity: 278 each. FOB destination to Elmendorf AFB, Alaska. Appliances are to be delivered to each resident housing on base that is listed on the sheets provided. Delivery, removal and installation must be accomplished NLT fourteen (14) weeks after award. The statement of work is as follows: The contractor will be required to remove and replace appliances with new appliances identified by the attached checklist. The contractor will be required to have the proper craftsmen to install and operationally check new appliances. Removed appliances will be identified on a DD 1348-1A and delivered to DRMO for disposition. An accurate record of both removed and installed appliances will be recorded in a Microsoft Excel 2000 spreadsheet. The following is an outline of the procedures. New Appliance Instructions: 1. Provide new current year appliances complying with the attached specifications or greater. Provide all new hardware necessary to install the appliances. 2. Contract the housing occupants using the attached name and phone number list. Set up a scheduled dated and time to replace specified appliances within no more than a two-hour window. Should the resident fail to meet the appointment, re-schedule delivery until appliance replacement is made at no additional cost to the government. 3. Inspect the old appliance to confirm the serial and model number provided are accurate. Contact Steven Million 552-4295 for instructions should the numbers prove inaccurate. 4. Appliances will be delivered to the housing units as specified by the attached checklist. 5. Install and operationally check new appliances. 6. Date of installation will be written legibly and warranty information will be attached to the back of new appliances using a permanent marker and a paper protector. Warranty information will also be recorded on the excel spreadsheet required by item 7. The contractor will supply an inventory of new appliances identifying the type of appliance, manufacturer, model number, serial number, date installed, and the housing unit address in a paper spreadsheet (reference attachment 1, which will be furnished upon award), and computer file using Microsoft excel 2000 on a 3.5? disk in IBM PC format to 3 CES/CEOE and 3 CES/ CEH. Old Appliance Instructions: 8. Remove old appliances from housing units and prepare paperwork for turn in to DRMO. 9. It is the contractor?s responsibility to acquire turn-in document numbers for the removed appliances from Base Supply customer service unit phone 552-4111. 10. The contractor will fill a DD Form 1348-1A out with the number of specific appliances (examples of filled in forms will be furnished upon award). All appliances will be coded F for unserviceable repairable. Copies of the DD Form 1348-1A will be attached to each appliance, also copies will be turned into DRMO and 3 CES/CEOE. 11. Contact DRMO to schedule drop off in advance (phone 552-4950 speak with Mark or Herb). Palletize the appliance and shrink-wrap them to secure to the pallet. Deliver old appliances to DRMO at 1273 Vandenburg Ave. Elemendorf AFB. 12. The contractor will supply an inventory of old appliances identifying the type of appliance, manufacturer, model number, serial number, date removed, and the housing unit address in a paper spreadsheet and computer file using excel 2000 on a 3.5? disk in IBM PC format to 3 CES/CEOE and 3 CES/CEH. 12.603(c)(2)(iii) requires a statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27). The following provisions and clauses are incorporated by reference and shall remain in full force in any resultant contract. FAR 52.212-1 Instructions to Offerors, Commercial Items; FAR 52.212-2-Evaluation-Commercial Items. Award will be made based on low price/technically acceptable offer. The government reserves the right to make award on the initial quote received without discussions. Each offeror shall include a completed copy of the provision at FAR 52.212-3 Offeror Representations and Certification-Commercial Items with its quote (http://farsite.hill.af.mil). Incorporate FAR 52.212-4-Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. However, for paragraph (b) of the latter clause, only the following FAR clauses apply; 52.203-6-Restrictions on Subcontractor so the government, with Alternate I; 52.219-8-Utilization of Small Business Concerns; 52.219-14-Limitations on subcontracting; 52.222-21Prohibition of Segregated Facilities; 52.222-26-Equal Opportunity; 52.222-35-Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; 52.222-36-Affirmative Action for Handicapped Workers with Disabilities; 52.222-37-Employment Reports on Disable Veterans and Veterans of the Vietnam Era; 52.222-41, 52.232-33-Payment by Electronic Funds Transfer-Central Contractor Registration. Offerors must be CCR registered (http://www.ccr2000.com). Incorporate DFARS 252.212-7001-Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items. However, for paragraph (b) only the following clause applies: 252.225-7007-Buy American Act-Trade Agreements-Balance of Payments Program.
