Description
It is the intention of this Statement of Work to obtain construction services at the Uniformed Services University of the Health Sciences (USUHS), 4301 Jones Bridge Road, Bethesda, MD 20814-4779. Awarded Contractor shall provide all labor, equipment, and supervision for two installations 1) a SeamTek Quartz Mortar Broadcast Flooring System in room B4143 @ USUHS, and 2) a SeamTek Double Broadcast System @ AFRRI for 8 rooms. Specifications are below. The installation will be done at 1) USUHS in room B4143, which is approximately 484 square feet; 2) AFRRI in rooms 3507, 3512, 3534, 3536 3537, 3540, 3541, 3542 approximately 1,628 square feet, and 3RD floor hallway #3535 approximately 443 square feet. All rooms shall be completed with the associated cove base. Contractor shall remove the existing flooring prior to the installation of the new epoxy resin flooring system. A grinding process for the new flooring system to properly adhere shall remove the existing flooring adhesive; Prepare concrete to "open" the surface pores by means of vacuum blasting or muriatic acid prep removing all contaminants or bond breaking substances, including but not limited to dust, curing compounds, coatings, sealers, oil, and grease. Any oil and grease not removed by vacuum blasting must be chemically removed. Scrabbling or chipping hammers shall mechanically remove all delaminated or deteriorated floor surfaces. Areas to be patched should cut to a minimum of 1/2" deep at the perimeters and keyed to the existing concrete; Cracks and non-expansion joints less than 1/6" wide after surface preparation shall be routed or raked out and filled using a semi-rigid epoxy system. These filled cracks or joints shall have a fiberglass cloth or flexible urethane "band-aid" applied over them to further inhibit the cracks from telegraphing through the epoxy floor. If control joints are cracked through the slab and additional movement is anticipated, consult Design Professional or Engineer and the manufacturer for specific remedial action. Contractor shall provide and install approximately 81 linear feet of epoxy mortar quartz broadcast cove base and approximately 475 linear feet of double broadcast cove base. The cove base shall be an integral part of the flooring system. The cove base shall be 4" high. The floor surface and the cove base shall be installed at the same time. The installation shall be done in such a way that the flooring surface and the cove base are one unit and form a watertight seal over the entire floor surface including the cove base. An epoxy-flooring specialist shall perform installation with skilled mechanics having not less than three years of satisfactory experience in the application of the type and complexity of system as specified. The manufacturer of the Mortar Quartz Broadcast Flooring System and Double Broadcast Flooring System shall approve epoxy-flooring specialist in writing; all materials that make up the Mortar Quartz Broadcast Flooring System and Double Broadcast Flooring System shall be obtained from a single manufacturer. Each component application of the Flooring System will be in compliance with the manufacturers installation instructions including mixing and application methods, no hazardous odors for adjacent occupied areas, (i.e., rooms, hallways). At the Government's direction and under the Contracting Officer's Representative supervision the industrial epoxy flooring specialist shall take two random cores through the system to the substrate, prior to applying the seal coat to verify proper floor thickness. The epoxy floor specialist shall repair these areas to match the surrounding surface prior to applying the final seal coat. Any areas less than the specified thickness shall be removed and replaced by the installing epoxy floor specialist in a manner that does not affect the aesthetics or integrity of the overall floor; The total system thickness will be 1/8" minimum. The finished surface shall be impervious to water, be dense, be non-porous, and have a non-skid surface. Prior to application, cracks are to be treated with semi-rigid epoxy. Patching is to be performed prior to the flooring system installation. Primary system materials will be shipped and delivered in the manufacturers undamaged, unopened containers. Each container will be clearly marked with the following: Product Name; Manufacturers Name; Component Designation (a, b, etc.); Ratio of Component Mixture; Quantity of Contents. The epoxy-flooring specialist shall promptly inspect all direct job site deliveries to ensure that quantities are correct and that materials comply with requirements and are not damaged. The Contractor shall be responsible for all materials furnished by contractor's shop and shall replace all material that is found to be damaged/defective in manufacturing, transit, handling, or storage at no cost to the Government. Materials will be stored in accordance with manufacturer's specifications. All seals and labels will be intact and legible. Temperatures will be kept between sixty (60) degrees Fahrenheit and ninety (90) degrees Fahrenheit. Do not use materials, which have been stored for a longer period of time than the manufacturer's recommended shelf life. The Contractor shall visit job site prior to beginning the application of the Mortar Quartz Broadcast Flooring System and Double Broadcast Flooring System to evaluate slab condition, including slab moisture content and the extent of repairs required. Concrete sub-floors shall be tested to verify that the moisture content of the slab does not exceed that as recommended shelf life. The epoxy-flooring specialist shall exercise care during surface preparation and system application to protect surrounding substrates and surfaces as well as in place equipment. The epoxy-flooring specialist shall at his discretion as to the physical means used for preparation and protection. The epoxy floor specialist shall furnish a standard guarantee Mortar Quartz Broadcast Flooring System and Double Broadcast Flooring System for a period of one year after installation is complete. This labor guarantee shall include loss of bond and/or wear-through to the concrete substrate under normal wear and tear conditions. Not included in the guarantee is damage due to structural design deficiencies, including but not limited to, slab cracking due to lateral, rotational, or vertical movement, and gouging or other damage done by equipment, moisture in or under the substrate, Acts of God or causes not related to the system materials. Contractor shall submit manufacturers specification or specific products of the Mortar Quartz Broadcast Flooring, and Double Broadcast Flooring, including physical properties and performance properties and submit all Material Safety Data Sheets. Each component of the system will be evaluated on the basis of these standards. Manufacturers standard color chart shall also be submitted, and must afford the government a color selection from at least 12 standard colors. The epoxy-flooring specialist shall submit a system sample for verification purposes and finish texture approval. All work shall be performed in occupied areas. Care must be taken during all operations to protect employees passing through the area. All areas shall be kept clean and safe. Provide temporary dust covers, barriers, and signs to protect areas surrounding the work site. All work areas shall be cleaned daily. All trash and debris shall be picked up and removed from work site. Hallways shall remain accessible at all times. At completion of work all areas shall be left in a broom swept condition. All work shall take place during normal working hours, 7:30 AM to 4:00 P.M., Monday through Friday. This contract shall be performed from date of award through 30 December 2001.
