Description
This is a combined solicitation/synopsis for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. This solicitation (674-01-21 is issued as a request for quotations. Award will be made on 1) Technical qualifications, 2) Past performance and 3) Price, listed in descending order of importance. The solicitation document and incorporated provisions and clauses are those in effect through FAC97-27. The Central Texas Veterans Health Care System (CTVHCS) intends to procure the following services for the period October 1, 2001 through September 30, 2002 and two option years: Holmium YAG Surgical Laser System with certified medical laser safety officer (MLSO) to operate system within operating suite at this medical center. Technicians must be industry certified and have the MLSO certification in order to operate the equipment within the CTVHCS OR suite. Prices are requested on a per-case basis, with an anticipated 60 cases per year. Company will be expected to provide laser, set up all urology probes and laser signage, provide laser fibers for urological and arthroscopic procedures, deliver, install and set-up equipment, provide current preventive maintenance verification by laser system manufacturer, and registration of all equipment with the Bureau of Radiation Control. Offerors responding to this solicitation must submit a proposal no later than September 14, 2001 in order to be considered. Proposals should include information relating to the above evaluation factors, as well as pricing information. Please include literature on the specific Holmium YAG laser proposed to be used. This solicitation incorporates the following clauses: 52.212-1, 52.212-2 EVALUATION COMMERCIAL ITEMS, INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS, COMMERCIAL ITEMS, WHICH MUST BE COMPLETED AND SUBMITTED ALONG WITH PROPOSAL. In accordance with 52.212-4 contract terms and conditions commercial items, far 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (8/2000), the following clauses are incorporated by reference in paragraph (b); 52.222-26 EQUAL OPPORTUNITY (EE11246); 52.222-35 AFFIRMATIVE ACTION FOR SPECIAL DISABLED VETERANS AND VETERANS OF THE VIETNAM ERA(38USC 4212); 52.222-36 AFFIRMATIVE ACTION FOR HANDICAPPED WORKERS (29 USC 793); 52.22237 EMPLOYMENT REPORTS ON DISABLED VETERANS AND VETERANS OF THE VIETNAM ERA; 52.232-33 PAYMENT BY ELECTRONIC FUNDS TRANSFER CENTRAL CONTRACTOR REGISTRATION (5/99). ALSO INCLUDED IS FAR 52.237-7 INDEMNIFICATION AND MEDICAL LIABILITY INSURANCE (1/97). Contractor will maintain liability insurance issued by a responsible insurance carrier of not less than the following amount per specialty per occurrence: $1,000,000. All responsible sources may submit a proposal which shall be considered.
