Description
This is a combined solicitation/synopsis for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. This solicitation (674-01-45 is issued as a request for quotations. Award will be made on 1) Technical qualifications and ability to provide requested response time, 2) Past performance and 3) Price, listed in descending order of importance. The solicitation document and incorporated provisions and clauses are those in effect through FAC97-27. The Central Texas Veterans Health Care System (CTVHCS) intends to procure the following services for the period October 1, 2001 through September 30, 2002 and two option years: Cardiac perfusionist and autotransfusionist services to include provision of equipment and disposables, for intraoperative cell salvage during surgery at the Temple, Texas facility. The perfusionist/autotransfusionist must be trained and certified by the manufacturer of the autotransfusion equipment, which is to be provided by the company selected. The perfusionist/autotransfusionist will perform autologous blood salvage and provide blood management services in accordance with JCAHO, OSHA and other oversight agencies as appropriate. Perfusionist must be certified by the American Bard of Cardiovascular perfusionist, must provide professional liability insurance of $1,000,000. Procedure guidelines are those issued by JCAHO, AABB, CLIA and conform to all federal and state statues governing perfusion services. Regular services will be scheduled 24 hours in advance; however, contractor must provide emergency services within one hour of notification. Estimated requirements are six cases per month or 72 cases per year.0l. Offerors responding to this solicitation must submit a proposal no later than September 7, 2001 in order to be considered. Proposals should include information relating to the above evaluation factors, as well as pricing information. This solicitation incorporates the following clauses: 52.212-1 INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS, 52-212-2 EVALUATION COMMERCIAL ITEMS, 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS, COMMERCIAL ITEMS, WHICH MUST BE COMPLETED AND SUBMITTED ALONG WITH PROPOSAL. In accordance with 52.212-4 contract terms and conditions commercial items, far 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (8/2000), the following clauses are incorporated by reference in paragraph (b); 52.222-26 EQUAL OPPORTUNITY (EE11246); 52.222-35 AFFIRMATIVE ACTION FOR SPECIAL DISABLED VETERANS AND VETERANS OF THE VIETNAM ERA (38USC 4212); 52.222-36 AFFIRMATIVE ACTION FOR HANDICAPPED WORKERS (29 USC 793); 52.22237 EMPLOYMENT REPORTS ON DISABLED VETERANS AND VETERANS OF THE VIETNAM ERA; 52.232-33 PAYMENT BY ELECTRONIC FUNDS TRANSFER CENTRAL CONTRACTOR REGISTRATION (5/99). ALSO INCLUDED IS FAR 52.237-7 INDEMNIFICATION AND MEDICAL LIABILITY INSURANCE (1/97). Contractor will maintain liability insurance issued by a responsible insurance carrier of not less than the following amount per specialty per occurrence: $1,000,000. All responsible sources may submit a proposal which shall be considered.
