Description
This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with the additional information included in this notice. This solicitation number is DADA10-10-T-0150 and is issued as a Request for Quotation (RFQ). This procurement is 100% Small Business Set Aside. The North American Industry Classification (NAIC) Code is 611710 and the Small Business Standard is $5M. The anticipated closing date is within 10 calendar days after RFQ publish date in the Commerce Business Daily (CBD), approximately July 20, 2001. The provision at FAR 52.212-1, Instructions to Offerors-Commercial, with addenda, FAR 52.212-4, Contract Terms and Conditions-Commercial Items , without addenda, and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items without addenda, apply to this acquisition. The contractor shall serve as the Trauma Management Coordinator for the Defense Medical Readiness Training Institute (DRMTI), Fort Sam Houston, Texas. The contractor shall be responsible for organizing, implementing and designing the Advanced Trauma Life Support (ATLS) Course, the Basic Trauma Life Support (BTLS) Course, the Trauma Nurse Core Course (TNCC), the Pre-Hospital Trauma Life Support (PPTHLS) Course, and the Advanced Burn Life Support (ABLS) Course. The contractor shall be responsible for reviewing and revising, as needed, the course policies for DMRTI instructors, coordinators, and affiliate faculties. Contract period shall be a base period plus two one-year options. Basic contract period is from August 1, 2001 through July 31, 2002. Contractor qualifications shall include (1) Master's degree in emergency or critical care nursing from an accredited school with a minimum of one-year experience in a trauma setting; (2) The contractor shall possess sound leadership skills, managerial experience, excellent interpersonal skills, and effective oral and written communication skills in the English language. To be considered qualified, the contractor shall have knowledge of (1) Standards, course content and requirements for ATLS, ABLS, PHTLS, TNCC, and Basic Trauma Life Support (BTLS); (2) Protocols and procedures for pass and fail status for ATLS, ABLS, PHTLS, BTLS and TNCC; (3) Must be an instructor in ABLS, TNCC, and BTLS (TNCC INSTRUCTOR/TRAINER PREFERRED). Offeror must be familiar with the details of conducting an instructor course for each of the professional programs. Contractor shall possess knowledge of (1) Microsoft Office Programs (Word, Excel, Access, Power Point)to assist with data retrieval for all course and computerization and format courses content into Power Point slides on CD-ROM for all professional courses; (2) The organizational structure of DRMRTI, work methods, and goals of the organization; (3) Health service support elements required to support combined operations with emphasis on joint service compatibility; (4) The duties and responsibilities of an instructor and a Trauma Management Coordinator. The contractor's proficiency in these areas can be shown by previous successful experience in a U.S. Army Training and Doctrine Command (TRADOC/Army Medical Department (AMEDD) School offering similar type courses. Offer shall provide a resume to reflect education, experience, and past performance and include price for 12 months for each contract period. Offer shall provide at least three references for past performance verification by the contracting office to include the contract number, period of contract, contract total, point of contact at contracting office, telephone and fax numbers, and eMail address, if available. Contractor experience shall reflect that (1) Contractor has monitored classes and submitted written comments to approving official; (2) Contractor has performed analysis of statistics on course attendance and pass/fail rates and instructor information to develop detailed and comprehensive plan for future course schedules; (3) Contractor has functioned as a reference for inquiry from course instructors and provided guidance to finding reference material; (4) Contractor has functioned as the department of training web page designer; (5) Contractor is familiar with web page authoring tools such as MS Front Page and Web Page Design Principles; (6) Contractor has analyzed programs that go over budget and develop plans to correct budgetary problems; (7) Contractor has instructed at least ten (10) resident TNCC Courses and two (2) non-resident classes. The contractor has been responsible for all pre-course, course, and post-course preparation and reports as required. Offerors shall include a completed copy of the provision at FAR 52.212-3, ALT I, Representations and Certifications-Commercial Items with offer. Interested offerors may request a copy of Request for Quotation (RFQ) and shall include company name, point of contact, telephone and fax number, Email address, cage code, DUNS number, tax identification number, central contractor registration number, and correct mailing address.
