Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written proposal is requested. THERE WILL BE NO WRITTEN REQUEST FOR QUOTATION ISSUED. The solicitation document and incorporated provisions and cclauses are those in efffect through FAC 97-27. This solicitation is issued on an unrestricted basis. The VA Medical Center Asheville, North Carolina has a requirement for one each Vascular Ultrasound Unit. The unit shall have the following capabilities: -Image Bookmarking -Longview-Longitudinal measurements -Pullback sled-Precise pullback for measurements and analysis. -Digital Image storage -Patient Database -Pre/Post intervention analysis. The equipment shall be Boston Scientific, Galaxy IVUS Console or equal. Equipment shall be new. Proposals for reconditioned or used equipment shall not be accepted. Proposals for other than the Boston Scientific Galaxy IVUS console shall provide detailed specifications of the equipment proposed as an or equal. Offeors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror-Commercial Items. As stated in this clause, the Government intends to evaluate offers and award a contract without discussions with offerors, therefore the offeror's initial offer should contain the offeror's best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. OFFERORS SHALL INCLUDE A COMPLETED COPY OF FAR CLAUSE 52.212-3, OFFEROR REPRESENTATION AND CERTIFICATIONS. An offeror shall also include its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS Number) and Tax Identification number. The resulting firm fixed-price contract award will incorporate the requirements of the clauses at FAR 52.212-4, Contract Terms-Commercial Items, FAR 52.212-5, Contract Terms and Conditions required to Implement Statutes or Executive Orders-Commercial Items.
