Description

Provide facility, labor, and material for eyeglasses and related services as per the statement of work and the terms and conditions of the contract. The contractor shall furnish prescription eyeglasses to include frames and lenses. Contractor shall be required to provide the measuring pupillary distance, near and far, sizing for eyeglass frames, verifying prescriptions for optifcal lenses, adjust finished eyeglasses, oversee the placement receipt and delivery of prescription eyewear, complete required paperwork, and accomplish repairs to provided eyeglasses for patients of the VA Salt Lake City Health Care System. The contract will be a multi-award contract. The areas of service will be in the state of Utah, Elko and White Pine counties of Nevada; Clark, Fremont, Jefferson, Madison, Teton, Bonneville, Bingham, Power, Bannock, Caribou, Oneida, Franklin, and Bear Lake Counties of Idaho; and Teton, Sublett, Lincoln, Uinta and Sweetwater Counties of Wyoming. The contract will be a fixed-price commercial contract. The contract will run from the date of award through May 31, 2002 with four one-year options to extend the contract. The issue date will be on or around May 4, 2001. The BOD date will be on or around June 5, 2001.
