Description
PRESOLICITATION: Work to be done is to construct a 4259 square foot addition to the Tuba City Dental Clinic,Tuba City, Arizona and renovate the Clinic's existing 3526 square foot facility to make modifications in the existing interior of the Tuba City Indian Medical Center including the Eye Clinic area, CT/Mammogram Room and Urology Room. The proposed contract listed herein is set aside under the Buy Indian Act for a MINIMUM 51 PERCENT INDIAN OWNED, CONTROLLED, AND OPERATED FIRM. In accordance with the Public Health Service Acquisition Regulation (PHSAR) Paragraph 380.503(e), under a 51 percent Buy Indian Set-Aside, not more than 50 percent of the work to be performed under a prime contract awarded pursuant to the Buy Indian Act shall be subcontracted to other than Indian firms. For this purpose, the work to be performed does not include the provision of materials, supplies, or equipment. In order for an Indian firm to prove they meet the qualifications requirement of the Buy Indian Set-Aside restriction, i.e., offerors who are currently certified by the Indian Health Service (IHS) or the Bureau of Indian Affairs (BIA) as an "Indian Firm", need to submit their certification (Tribal Affiliation and Degree of Indian Blood) with their bid. Offerors who are not currently certified by IHS or BIA are required to submit proof of ownership and eligibility for Indian preference under the "Buy Indian Act". Joint ventures will be acceptable as long as the Indian owned firm is the managing entity. Estimated contract period is 365 calendar days. Requests for more than one set of plans and specifications for 51 percent Indian owned firms MUST be accompanied by a NON-REFUNDABLE FEE (Certified Check or Money Order) made payable to the Indian Health Service in the amount of $150.00 for each set ordered. All solicitation documents for NON-INDIAN firms are $150.00. All responsible sources may submit a bid that shall be considered by the agency. Bid opening date is scheduled O/A June 11, 2001, at 2:00 pm (Dallas Time).
