Description
Contractor to provide Optometry services in accordance with Public Law 104-262 and 38 USC 8153 in order to support the VA Roseburg Healthcare System, Roseburg OR 97470. Contractor must have an established office within the City Limits of Roseburg, Oregon 97470. Reimbursement will be made using the current (PAR) Medicare Physicians Fee Schedule as defined by Current Procedural Terminology (CPT) codes and applicable Health Care Financing Administration (HCFA) Common Procedure Coding System (HCPCS), for Optometry Services, in place at the time service is rendered. For the purpose of this contract, reimbursements will be based on the locality code used by Medicare for the contractor's location. Periodical and annual updates to the schedules are incorporated by reference and will become effective as directed by the publication. Definitions of CPT Codes: CPT Code 99203 New Patient Detailed Eye Exam will include but is not limited to: detailed history, detailed examination and decision making considered at least "low complexity". CPT Code 99214 Established Patient Detailed Eye Exam will include but is not limited to: detailed history, detailed examination and decision making considered at least "moderate complexity" (At least two of the three criteria are met to satisfy the Medicare definition). CPT Code 92015: Determination of refractive state. Contractor to provide quality professional vision care services for the veteran patient population authorized by the VA Roseburg Healthcare System. These services shall be provided to pre-approved, eligible veterans located in southern Oregon. Contractor shall provide a comprehensive examination that shall include refractive services for spectacle prescriptions and evaluation for eye disease. The contractor will utilize diagnostic pharmaceutical agents for examination of the eyes. In certain instances, therapeutic agents can be prescribed (see patient referral below). Contractor will provide a written prescription for eyeglasses, when indicated, This prescription can be used at participating VA optical dispensers but is valid at other locations. The contractor shall be licensed in accordance with the applicable laws of the State of Oregon and have scope of practice credentials. Standard of care shall be in accordance with or exceed the American Optometric Association (AOA) optometric clinical practice guidelines. Contractor shall submit a copy of the license, to be reviewed for technical qualifications, for each respective optometrist providing service. Contractor shall submit as copy of the license for any new provider hired throughout the term of the contract. Solicitation packages will be available after 4/9/01.
