Description

Madigan Army Medical Center, Tacoma, Washington requires laboratory screening services for the Newborn Nursery. The contractor shall provide all transportation, labor, supplies, equipment and test kits and perform the following tests: Phenylketonuria, Congenital Hypothyroidism, Galactosemia, Maple Syrup Urine Disease, Biotinidase, Congenital Adrenal Hyperplasia, Hemoglobinopathy. The laboratory will complete the testing and notify the hospital of the results in accordance with the specifications set forth in the Request for Proposals. The laboratory shall be accredited by the College of American Pathologists (CAP), be a state or federal regional reference laboratory, or be licensed under the Clinical Laboratory Improvement Act of 1968. Approximately 4600 test kits are required per year. It is anticipated that a firm fixed price base year plus four one year option periods, requirements type contract will result from this solicitation. The solicitation number is DADA13-01-R-0003. The NACIS code is 621511 and the business size standard is $5 million. Performance expected to begin 01 June 2001 through 30 November 2001 for the base year with four option periods.
