RFQ No. F04684-01-T0921 Page 1 of 1 PURCHASE ORDER F04684- 28 Mar 2001 This is a combined synopsis/solicitation for commercial services prepared in accordance with Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented, with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The Solicitation Number is F04684-01-T0291. This is a request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-23 and DFARs Change Notice (DCN) 20001213. This is a 100% set-aside for small business participation only. The North American Industry Classification Systems (NAICS) is 541380. The Small Business Size Std. is $5,000,000.00. This acquisition consist of Line Item 0001: Services Non-Personal: Provide all labor, tools, materials, equipment, facilities, and supervision necessary to perform Dissolved Gas Analysis of Transformers at Vandenberg AFB in accordance with Statement of Work dated 9 MAR 01. (Attachment #1) Transformer location and sizes dated 26 Feb 01 (Attachment #2), and Wage Determination No.: 94-2064, Revision 11 dated 09/13/2000 (Attachment #3). All oil samples shall be collected within forty five (45) calendar days of award of contract, and required reports shall be submitted within forty five (45) days of completion of testing/analysis for a total period of performance of ninety (90) days. The following provisions and clauses apply. Provision 52.251-1 Solicitation Provisions Incorporated by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make the full text available. Provision 52.212-1, Instructions of Offerors-Commercial, FAR 52.212-2 "Evaluation-Commercial Items" applies to this acquisition. Insert in Para (a), "The following factors will be used to evaluate offers, (1) price and (2) past performance." Offerors must submit with their quotes a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items." The clause at FAR 52.212-4, "Contract Terms and Conditions-Commercial Items," applies to this acquisition.
