Description

This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation number N00181-01-Q-0205 is issued as a request for quote (RFQ). Quote responses are acceptable via mail or by fax (757) 396-8503. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-21. SIC 3841 NAIC 33911 with a standard size of 500 employees applies. This requirement is for a fixed price contract for the following line items: CLIN 0001: Cover, Lens, Mine Safety Appliance P/N 88379, Quantity 500 EA.CLIN 0002: Cover Lens, Ultra View, Mine Safety Appliance P/N 456975, Quantity 100 EA.CLIN 0003: Kit, Spectical, Mine Safety Appliance P/N 454819, Quantity 20 EA CLIN 0004: Spring for Ultra Vue, Mine Safety Appliance P/N 87013, Quantity 50 EA. CLIN 0005: Spacer, Mine Safety Appliance P/N 87009, Quantity 50 EA. CLIN 0006: Lens, Mine Safety Appliance P/N 96677, Quantity 500 EA CLIN 0007: Filters, Mine Safety Appliance P/N 815180, Quantity 400 EA CLIN 0008: Filters, Mine Safety Appliance P/N 815176, Quantity 200 EA CLIN 0009: Filters, Mine Safety Appliance P/N 815998, Quantity 60 EA Inspection/Acceptance at destination, NNSY, Receiving Officer, Bldg. 276, Portsmouth, VA 23709-5000. Delivery: FOB Destination, Norfolk Naval Shipyard, Portsmouth, VA. Delivery Date: 1 March 2001. The provision at FAR 52.212-1, Instructions to offers-Commercial items applies. FAR 52.212-2, Evaluation-Commercial Items, applies. "ADDITIONAL EVALUATION FACTORS CONTRACTOR EVALUATION SYSTEM, RED/YELLOW/GREEN PROGRAM (MAY 1992)" will be used by the Contracting Officer to assist in determining the best value for the government price, past quality performance, and other factors considered. FAR 52.212-3, Offeror Representation and Certification -- Commercial items shall be completed and returned with offer along with descriptive literature for item offered. FAR 52.212-4, Contract Terms and Conditions -- Commercial Items applies to this acquisition. FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items apply. FAR 52.212-5 (b) (1), (5), (11), (12), (13), (14), (15), (16), (17) and (22) apply. DFAR 252-212-7001 apply to this acquisition.
