Description
Provide Family Nurse Practitioner/Physician Assistant services (1 positions) at the Coast Guard clinics located at Ketchikan, AK to beneficiaries referred for treatment by a Coast Guard Medical Officer. This will be a personal service contract. This requirement will be accomplished utilizing Best Value Commercial Item simplified acquisition procedures. The contractor will work weekdays during hours established by the Clinic Supervisor, normally not to exceed 8 hours a day, 40 hours a week for a total of 1920 hours per year. Period of performance will be on or about 15 May 2000 or date of award through 14 May 2001 with four option years. The Request for Proposals will be issued on or about 16 February 2001 and close on or about 29 Mar 2001. This procurement is a Small Business Set-Aside. The NAICS code for this acquisition is 62134. The SIC code is 8049 and the small business size standard is $5.0 million average annual receipts for the proceeding four fiscal years.
