PURCHASE SERVICES, NON-PERSONAL: -- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. The procurement is to provide all labor, equipment, tools, materials, electricity, transportation, and supervision necessary to perform post mitigation radon testing on 50 homes and 33 homes that have been mitigated and must have post mitigation sampling, at Scott AFB IL in accordance with the attached Statement of Work (SOW). Inspection and acceptance will be at Scott AFB IL. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. This solicitation, reference number is F7390403130100 issued as a Request for Quotation (RFQ). This solicitation is set-aside 100% for small business. The North American Industry Classification System Code (NAICS) is 541380 the small business size standard is 5 million dollars. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. Vendor must be actively registered with Central Contractor Registration. Offerors must comply with all instructions contained in FAR 52.212-1, Instructions to Offerors-Commercial. FAR clause 52.212-2, Evaluation-Commercial Item applies. Award of this acquisition will be based on Best Value to the Government and the evaluation criteria for award will be based on past performance and price. If proposal is acceptable, past performance and price will be evaluated. Offerors will provide a list of three (3) of the most relevant (like or similar) services performed for either the Federal Government or commercial customers within the last three years. Past Performance references must be submitted along with price quote. The following additional FAR provisions and clauses do apply: FAR 52.212-3 Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer.
