Description
PSC -- B537 NAICS -- 621511 MEDICAL TEST LABORATORIES. FISC Norfolk by the authority of FAR Part 12.6 intends to award a simplified acquisition, firm fixed price contract to The Commonwealth of Virginia for Newborn Infant Screening Kits. The acquisition of the kits is mandated by the state of Virginia and will be used for the evaluation and diagnostics of genetic disorders. Each kit shall include the following for the testing of 10 newborns: filter/litmus paper collection devices, lab chits, envelopes to return the blood samples to the Commonwealth of Virginia, information pamphlets for the parents, laboratory evaluation, and diagnosis services of the collected data for 7 disorders. The resulting period of performance will be from the date of award to 30 September 30, 2001 with priced option provisions to extend the term for four one-year options in addition to the provision of FAR 217-8 " Option to Extend Services. Potential Offerors are hereby notified that the solicitation will ONLY be available by downloading the documents at FISC Norfolk Website: http://www.nor.fisc.navy. mil/contracts/ solicitations/default.htm or at http://neco.navy.mil. This office will no longer issue hard-copy solicitations. By submitting a proposal, the vendor will be self-certifying that neither the requestor nor the principal corporate officials and owners are currently suspended, debarred, or otherwise ineligible to receive contracts from any Federal Agency. Failure to do so may represent grounds for refusing to accept the proposal. Number Note 22 applies.
