Description

This notice is provided on a sole source basis, with an intent to award a firm fixed price order with QUEST DIAGNOSTICS. This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This solicitation number, N00259-01-T-0006, is issued as a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in efect through FAC 97-19. Clin 0001: The following are the necessary tests to be performed: VARIOUS LAB TESTS, (please contact Domingo B. Custodio, Jr. for a lists of tests at (619) 532-8106) for the period of 01Nov00 through 30Nov00. Unit of Issue: LOT, Quantity: 01. This solicitation will be evaluated based on price, technical capability, and delivery. This acqusition incorporates the following FAR clauses: 52.212.1 INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS (Oct 95); 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS-COMMERCIAL ITEMS (Oct 95) must be completed and submitted along with the proposal (please contact Domingo B.
