Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR), Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and is being issued as a Request for Quotations (RFQ) and a written solicitation will not be issued. The solicitation document and incorporated clauses and provisions are those in effect through Federal Acquisition Circular 97-19 and Veterans Affairs Acquisition Regulation (VAAR), VA Acquisition Circular 97-8. The VA Consolidated Mail-Out Pharmacy (CMOP), located at 2962 South Longhorn Drive, Lancaster, Texas 75134) anticipates a sole source award of a firm fixed price contract to Automed, Technologies, Inc, 875 Woodlands Pkwy, Vernon Hills, IL 60061 for maintenance/phone support for two CMOP owned Automed/ Optifill systems. System I is approximately 5 years old and system II is approximately 3 years old. The contract award shall be for the period, date of award of the contract through 09/30/2001 and will be based on lowest responsive and responsible offer received that is in the best interest of the government. The contract shall provide for: (1) hardware and software technical phone support within 2 hours of notification, support hours 7:00A.M -- 9:00 P.M. local time Monday through Friday excluding federal holidays; (2) hardware and software technical phone support for after hours, weekend and holidays; (3) preventive maintenance; and (4) priority on-site services. Offerors are required to provide a monthly price for normal hours phone support, hourly rates for phone support after hours, weekend and holidays, and for on-site service. Offerors shall also provide the company agreement for those services, and a completed FAR clause 52,212-3 (Representations and Certifications). FAR clauses and provisions incorporated herein by reference are FAR 52.212-1, 52.212-2, 52. 212-3, 52.212-4, 52.212.5 and addenda, 52.217-8, 52.233-2, 52.233-3, 852.233-70 and 852.270-4. These clauses and provisions are available on the Internet web at http://www.arnet.gov/far. Those interested parties who are able to provide the requested services are encouraged to submit a quotation. The offer must be received on or before 2:30 PM Dallas, TX local time on 10/25/2000. Offers must reference the solicitation number and be sent to the attention of the Contracting Officer.
