The VA Medical Center, Miami, Florida intends to negotiate a sole source contract with Medtronic A.V.E. the Request for Quotation RFQ# 546-99-07 entitled "GFX-2 Over the Wire Coronary Stents" is hereby issued for commercial items and is prepared ion accordance with the format Subpart 12.6. The SIC Code is 3814 and the small business size standard is considered 500 employees. This CBD announcement constitutes the only solicitation issued. A separate written solicitation document will not be issued. Must be FDA approved for stenting De novo (abrupt closure) Native Coronary Arteries. Medtronic A.V.E. shall provide over the-wire coronary stents available in multiple sizes as indicated: 3.0 x 8mm to 4.0 x 8mm, 3.0 x 12mm to 4.0 x 12mm, 3.0 x 16mm to 4.0 x 16mm, 3.0 x 18mm to 4.0 x 18mm, 3.0 x 24mm to 4.0 x 24mm and 3.0 x 30mm to 4.0 x 30mm. These stents shall provide moderate radiopacity in order to optimize visualization of stent under fluoroscopy for accurate stent placement. Stents shall be of such design and construction as to offer optimum strength and flexibility. Delivery terms shall be FOB Destination to the VA Miami, Florida, Warehouse 90D, 1201 N.W. 16th Street, Miami, Fl 33125.
