G -- TRANSITIONAL SUBSTANCE ABUSE TREATMENT SERVICES Provide outpatient assessment and counseling services for substance abuse treatment for male/female Federal offenders in the Waseca, Faribault and Nicollet County area(s) in south central Minnesota, held under the authority of various United States Statues. A Firm-Fixed Price-Requirements type contract with a two year base period and three one-year options is anticipated. Services for the base period is scheduled to start October 1, 1999, through September 30, 1999, and shall include 6 inmates as follows: Intake Assessment (one per person); Individual Counseling (96-half hour sessions per inmate); Group Counseling (144-half hour sessions per inmate). Services for options years one, two, and three shall include services for 6 inmates as follows: Intake Assessment (one per person); Individual Counseling (48-half hour sessions per inmate); Group Counseling (72-half hour sessions per inmate) for option year one, two and three respectively. The first option year will be October 1, 2001 through September 30,2002; the second option year will be October 1, 2002 through September 30, 2003; the third option year will be October 1, 2003 through September 30, 2004. All services shall be performed in accordance with the Statement of Work entitled Transitional Drug Abuse Treatment (TDAT) Substance Abuse Treatment dated May 1999. Proper licensing in the State of Minnesota is required. The estimated issue date for the Request for Proposal is o/a May 24, 1999. The estimated closing date is June 24, 1999.
