Contractor to provide non-personal services in the specialty of Optometry, by a licensed Optometrist, to inmates confined at the Federal Correctional Institution in Loretto, Pennsylvania. Offerors must be licensed in the State of Pennsylvania. The qualified Optometrist will be responsible for, but not limited to, submitting written eyeglass prescriptions and providing written evaluations of patient's examinations. The work schedule shall normally consist of three (3) sessions per month. Each session is defined as approximately seven (7) to eight (8) hours, with the evaluation of ten (10) to fifteen (15) patients. The period of contract performance shall run from date of award through September 30, 1999, with options to extend the performance period for an additional four (4) one-year periods.
