existing barcoding capabilities. PS Form 3849 measures 3-1/2"H x
5-1/2"W and will utilize a 16-digit barcode. The barcodes must measure
American National Standards Institute (ANSI) grade A or B, guidelines
X3.182-1990. A minimum of 600 DPI is required. The barcode elements
are: Code Set C, two digit product code-52, three digit printer ID
code, nine digit sequential number, MOD 10 check digit, MOD 103 check
digit and stop code. A fifteen digit, human readable numeric
representation of the barcode must also appear and conform to American
National Standards Institute X3.17, 1981. It is estimated that 18
million forms will be ordered over a two (2) year period for the Great
Lakes Area (Illinois, Indiana and Michigan). Each form will be
sequentially numbered using standard Uniform Symbology Specifications
(USS)Code 128 (Subset C) barcode system. Prints: offset book, basis
size 25" x 38", 70 lbs per 500 sheets. Qualified suppliers can obtain
