combined synopsis/solicitation. Request for Quotation
#RFQ-618-121-98-LK entitled "Coronary Stent System" is hereby issued
for commercial items and is prepared in accordance with the format in
FAR Subpart 12.6 as supplemented by additional information included in
this notice and incorporates provisions and clauses in effect through
FAC 97-04. The SIC Code is 3841 and the small business size standard
is considered 500 employees. This CBD announcement constitutes the only
solicitation issued. A separate written solicitation document will not
be issued. Contractors' proposals are hereby being requested and
response to requirements listed below shall be submitted on
contractor's letterhead and must address all requirements listed below
and should contain the name, title and signature of person authorized
to submit proposals/bids on behalf of the contractor. Descriptive
literature for each item offered must also be included in proposal/bid
package. Representative samples may be requested by the Contracting
Officer at her discretion and will be due within 5 days of request. The
VA Medical Center (VAMC), Minneapolis, Minnesota, has a requirement for
an over-the-wire coronary stent system available in multiple sizes
ranging from approximately 3.0mm x 9.0mm through 4.0mm x 30.0mm. These
stents shall provide moderate radiopacity in order to optimize
visualization of stent under fluoroscopy for accurate stent placement.
Stents shall be of such design and construction as to offer optimum
strength and flexibility. It is believed that Arterial Vascular
Engineering (AVE) of Santa Rosa, California, the manufacturer of the
Micro Stent II System is the sole source for such a system. AVE has a
system offering 18 sizes of Micro Stents with patented design and
construction technology that offers enhanced flexibility, strength,
resiliency, durability and an edgeless design. Delivery terms shall be
FOB Destination (unless vendor states otherwise) to the VA Medical
Center, Warehouse (90D), One Veterans Drive, Minneapolis, MN, 55417.
This will be a firm fixed price contract. Prices bid will be in effect
for one year. Evaluations may be conducted to determine that products
offered will meet Government's requirements. Evaluation factors of
technical capability of the items offered and past performance history
(including case reports) will be considered along with price to
determine "best value" to the Government. Evaluation may be made with
or without discussions with the offeror(s). The following incorporated
FAR clauses and provisions are those in effect through FAC 97-04 and
