Lancaster Road, Dallas, TX 75216, has a requirement for one (1) Community Based Outpatient Clinic in each of the following Texas Counties: Wise, Baylor, Archer, Clay, Young, Throckmorton, Jack and Montague. The contractor shall provide for the delivery and management of continuous primary and preventive medical care for enrolled VA patients. The contractor shall provide health care providers, medical facilities, medical equipment and supplies, pharmaceuticals, and all administration functions in accordance with the terms and conditions of the proposed contract. Compensation will be made through a monthly capitation payment. Issue date will be on, or about, March 30, 1998. Proposal due date will be on, or about, 4/30/98.
