R -- EVALUATION OF THE ALZHEIMER DEMONSTRATION GRANT TO STATES PROGRAM SOL HRSA 240-BPHC-12(8) DUE 041098 POC Helene Braun, Contract Specialist, (301)443-5131 The Health Resources and Services Administration proposes to negotiate a contract on a sole source basis with the University of Kansas, Gerontology Center, to (1) support the continuation of data collection and monitor for accuracy, completeness and quality, the collection of all program data from the 15 Alzheimer's Demonstration Grant to States Program State grantees; (2) to utilize the data in (1) above, and other previously collected data, perform an analysis and prepare a report for the 1997 grant year; (3) receive technical assistance, coordination of data collection activities and monitoring for accuracy, completeness and quality, for the Managed Care Initiative; (4) receive consultation services for the HRSA/BPHC evaluation strategy; and (5) receive consultation services to the Project. Based on the above, the University of Kansas, Gerontology Center, is deemed the only responsible source capable of providing the required work and services as set forth in 41 U.S.C. 253(c)(1) -- only one responsible source.
