for commercial items prepared in accordance with the format in FAR
Subpart 12.6, as supplemented with additional information included in
this notice. This announcement constitutes the only solicitation;
proposals are being requested and a written solicitation will not be
issued. The solicitation # is 899-98-0001. This is a request for
quotation (RFQ). The solicitation document and incorporated provisions
and clauses are in effect through Federal Acquisition Circular 97-03.
(initial year of service), April 1, 1998 through March 31, 1999 --
DESCRIPTION: 2-eight cubic yard top loading containers, pick-up will be
twice a week, Quantity: 12, Unit: month, Price $__ ____, Total $__
____, and 1-thirty-five cubic yard capacity roll-off container pick-up
will be as needed (estimated six times per year), Quantity: 6, Unit:
each, Price $__ __&llmdash;_, Total $__ ____. There are options for two
additional years of service. Offerors, please submit price for optional
years as well. Items are same as above, but for the period April 1,
1999 through March 31, 2000 and April 1, 2000 through March 31, 2001
respectively. REQUIREMENTS: Contractor is to furnish all labor and
equipment to provide trash/refuse removal at the National Memorial
Cemetery of the Pacific (NMCP), Maintenance Area, 2177 Puowaina Drive,
Honolulu, HI 96813. Contractor is to provide two (2) eight cubic yard
capacity top loading containers and one (1) thirty-five cubic yard
capacity roll-off container at all times, containers should be in place
by April 1, 1998. Contractor will ensure the containers are rust free,
neatly painted, labeled as required and maintains a presentable
appearance at all times. Containers that are considered unacceptable by
the Contracting Officer's Technical Representative shall be replaced by
the contractor at no additional cost to the Government. Contractor will
be required to empty 2-eight cubic yard containers two (2) times a
week, Tuesday and Friday including all holidays, the thirty-five cubic
yard container will be emptied as needed (as called in by NMCP staff),
estimated 6 times per year. The containers must be emptied only between
the hours of 8:00am and 12:00pm (noon). If refuse is dumped on the
ground during pick-up the contractor will be responsible to pick-up
this refuse at own expense. Contractor shall ensure they are in full
compliance with all state and local codes when disposing of all refuse.
Contractor will ensure all safety requirements are followed when
servicing the containers. Any safety related discrepancies or hazards
noted by the Contractor when servicing should be corrected immediately
or reported to the VA for correction. Contractor will provide a 24
hour emergency number. Emergency service shall be provided within two
(2) hours of notification. Provision 52.212-1, INSTRUCTIONS TO
OFFERORS-COMMERCIAL,- COMMERCIAL ITEMS IS APPLICABLE. The Government
will evaluate offers for award purposes by adding the total price for
all options to the total price for the basic requirement. The
Government may determine that an offer is unacceptable if the option
prices are significantly unbalanced. Evaluation of options shall not
obligate the Government to exercise the option(s). Offeror must submit
a completed copy of provision 52.212-3, OFFEROR REPRESENTATIONS AND
CERTIFICATIONS-COMMERCIAL ITEMS with its quotation. Clause 52.212-4,
CONTRACT TERMS AND CONDITIONS is applicable to this acquisition. Clause
52.212-5, CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES
OR EXECUTIVE ORDERS -- COMERCIAL ITEMS, applies to this acquisition.
In addition, all applicable clauses and provisions from the VA
Acquisition Regulation apply. Quotations are due to the Contracting
