Q -- MEDICAL SERVICES SOL 98S036(N) POC Wanda Allison, Contract Specialist, (404) 842-6738 The Centers for Disease Control and Prevention (CDC) and the Agency for Toxic Substances and Disease Registry (ATSDR) located in Atlanta, Georgia are advertising the operation of the Occupational Health Service, Lifestyle Services and Employee Assistance Program Services. Currently the Occupation Health Services Contract services a Main Health Unit at the Clifton Road facility and an Auxiliary Health Unit at the Chamblee facility. The Main Health Unit responded to approximately 8500 visits during FY96 (10/1/95-9/30/96) and the Auxiliary Health Unit responded to approximately 2000 visits during FY96. Services to be provided include: Comprehensive occupational medical services to employees, visitors, and guest workers at CDC/ATSDR to include emergency medical care; pre-employment and in-service examinations; screening programs and medical surveillance; an immunization program; and health education services, blood tests, world-wide travel medicine, and response to on-the-job injuries and unique occupational hazards found in a research facility; Employee Assistance Program (EAP) with an 18% utilization rate during FY96; staffing of the Lifestyle Fitness Center at the Clifton Road facility from 6:30 am to 6:30 pm, Monday through Friday as well as a worksite health promotion program which included approximately 1000 participants during FY96. The contract period will include a 12-month base period with four 1-year renewal option periods. Issue date on or about March 10, 1998 with closing 30 days thereafter.
