(Site Code DADA16) The contractor shall provide Occupational Health
Services to approximately 142 Department of the Army (DA) and
Department of Defense (DOD) civilian personnel located at Pohakuloa
Training Area (PTA), Kilauea Military Camp (KMC) and Kilauea Military
Reservation (KMR), on the island of HAWAII (Hilo). The contractor's
professional, ancillary and administrative duties shall consist of
providing competent services specified in this contract. (1)Provide
outpatient Occupational medical care to DA/DOD civilian personnel,
Monday through Friday. (2) Occupational medical care will include but
not limited to the following services: (a) Complete Physical
Examinations (b) Nursing Health Appraisals/Limited Physical
Examinations (c) Follow-up Consultations/Referrals (d) Health
Education/Health Counseling (e) Treatment/Care for on the Job
Injuries/Illnesses (f) Immunization Requirements (g) Hearing
Conservation (h) Vision Conservation (i) Pregnancy Surveillance (j)
Chronic Disease Surveillance.(3)Contractor shall provide radiology,
laboratory, pharmacy, and other ancillary services necessary to support
Occupational medical services.(4) Provide appointment dates within five
(5) working days (5) Contractor shall not bill or invoice patients for
services rendered under this contract. (6) Provide acute care for
patients within 24 hours. (7) The contractor and any employee of the
contractor are not considered employees of the United States
Government. An estimated maximum of one (1) Medical Physician will be
required to perform the complete physical exam services. The
credentials of the Medical Physician shall be submitted with the
proposal: Medical Diploma, Internship Certificate, Residency
Certificate, Medical License and Liability Insurance. The contractor
shall furnish all personnel, equipment, repair parts, telephone
service, and supplies except as specified as government furnished.
Health care services provided shall be of a quality meeting or
exceeding current recognized national standards as established by the
American Medical Association (AMA), the Joint Commission on
Accreditation of Health Care Organizations (JCAHO) etc. The following

Convenience of the Government. This acquisition is UNRESTRICTIVE, RFT
DADA16-98-T-0110 is issued 02 Feb 98 and closed on 17 Feb 98 at 2;00 PM
(Hst). Proposed contract is for a firm fixed price requirement type
contract. Services shall commence approximately 01 Mar 98 through 30
Sep 98 with (4) option years to extend the contract. For any additional
