Synopsis #4063-96---This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number DAAH03-96-R-0030 is being issued as a request for proposal. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-39. This requirement will be procured on an unrestricted basis. SIC Code is 3695. CLIN 0001-Blank CD-ROMs with cardboard mailers; QTY. 15000 ea. CLIN 0002-Jewel Boxes; QTY. 100 ea; CLINs 0003-0057: blank CD-ROMs with cardboard mailers, option quantities of 1500 each. All options total 82,500, if exercised. CD-ROMs must be ISO-9660 compliant, blank, with gold backing, lacquer finish, and 650 MB/74 minute capacity. Bulk packaging is required in quantities of 75 per package on plastic spindles with plastic dome and screw-on nut to cover each spindle. Face (non-writing side) of CD-ROM must be blank to allow end user to fully utilize Rimage printers to imprint the appropriate command logo and specific data on the face of each CD-ROM after it is written. Cardboard mailers shall be provided for each CD-ROM. Delivery and acceptance is 30 days after award or exercise of option, as applicable. FOB Destination. Shipment will be to various CONUS locations. The provision at FAR 52.212-1, Instructions To Offerors-Commercial, applies to this acquisition. The provision FAR 52.212-2, Evaluation-Commercial Items applies to this acquisition. The following factors shall be used to evaluate offers (inclusive of options): Technical capability of the item offered to meet the government's requirement and price. Technical capability will be evaluated on a GO/NO GO basis. Offerors shall include product literature, pricing, and a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. FAR 52.212-4, Contract Terms and Conditions-Commercial Items, and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, apply to this acquisition.
