This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2025-06 EFFECTIVE 10/01/2025. The associated North American Industrial Classification System (NAICS) code for this procurement is 334510 with a small business size standard of 1250 employees. The NETWORK CONTRACTING OFFICE 5 is seeking to purchase a single non-contact ophthalmic biometry system with installation, training and warranty. Measurements must be obtained via non-contact. Measurements obtained via two-dimensional swept-source OCT (SS-OCT). Must have ability to obtain biometry readings on dense cataracts. Automated data transfer: Must be capable of seamlessly integrating with ophthalmic data management systems already in place to eliminate manual data entry errors. The data management system in place is the ZEISS FORUM. Must be capable of transferring data via ZEISS FORUM into the biometry unit and vice versa. DICOM compatibility: Must be DICOM-compliant to interface with electronic medical record systems i.e. VISTA. Physical and Operational Requirements Must have a compact and ergonomical design. Must include patient positioning features. Adjustable chin rest. Head support for stable measurements. Must be compatible with standard U.S. electrical outlets. Must be FDA-approved for use in ophthalmic biometry and comply with ISO standards for medical devices. Statement of Work 1.0 General 1.1 Background The mission of the Hershel Woody Williams VAMC is to provide patient care. The mission of the Optometry Service is to provide assessment, diagnosis, and treatment of vision care. This acquisition will be an additional piece of equipment that will take more accurate measurements used for cataract implants. 1.2 Objective Due to increased volume of cataract cases being performed, this acquisition will speed up the time spent doing manual A-scans, as well as give a more accurate measurement with the advanced technology the Argos will provide. 1.3 Scope of Work The Argos will replace having to do manual A-Scans. Must perform accurate biometry, keratometry and intraocular calculations including toric lenses, especially in difficult cataract cases. Measurements should be obtained via two-dimensional swept-source OCT (SS-OCT). Must obtain non-contact measurements. Must be able to dicom images into Vista Imaging and Zeiss FORUM. 1.4 Period of Performance N/A 1.5 Place of Performance The place of performance is the Eye Clinic, located on the third floor west of the Hershel Woody Williams VAMC. 1.6 Applicable Documents N/A 2.0 Specific Requirements/Tasks 2.1 Installation 3.0 Delivery or Deliverables The contractor will coordinate with the Veterans Affairs Medical Center department of biomedical engineering, to service all on-site installation of hardware and software, as well as to provide any on-going support within the frame of their warranty-period. The warranty contract shall be pursued and maintained by the department of biomedical engineering. 4.0 Meetings N/A 5.0 Required Travel and Other Direct Costs (ODCs) No travel will be required. 6.0 Government Furnished Property/Equipment/Information This device will be installed and used on existing property of the Hershel Woody Williams VAMC. 7.0 Special Instructions The C&A requirements do not apply, and that a Security Accreditation Package is not required 8.0 Glossary of Abbreviations and Acronyms N/A 9.0 Records Management Addendum RECORDS MANAGEMENT OBLIGATIONS A. � Applicability This clause applies to all Contractors whose employees create, work with, or otherwise handle Federal records, as defined in Section B, regardless of the medium in which the record exists. � B. � Definitions Federal record as defined in 44 U.S.C. § 3301, includes all recorded information, regardless of form or characteristics, made or received by a Federal agency under Federal law or in connection with the transaction of public business and preserved or appropriate for preservation by that agency or its legitimate successor as evidence of the organization, functions, policies, decisions, procedures, operations, or other activities of the United States Government or because of the informational value of data in them. � The term Federal record: includes [Agency] records.� does not include personal materials. applies to records created, received, or maintained by Contractors pursuant to their [Agency] contract. may include deliverables and documentation associated with deliverables. C. � Requirements Contractor shall comply with all applicable records management laws and regulations, as well as National Archives and Records Administration (NARA) records policies, including but not limited to the Federal Records Act (44 U.S.C. chs. 21, 29, 31, 33), NARA regulations at 36 CFR Chapter XII Subchapter B, and those policies associated with the safeguarding of records covered by the Privacy Act of 1974 (5 U.S.C. 552a). These policies include the preservation of all records, regardless of form or characteristics, mode of transmission, or state of completion.� In accordance with 36 CFR 1222.32, all data created for Government use and delivered to, or falling under the legal control of, the Government are Federal records subject to the provisions of 44 U.S.C. chapters 21, 29, 31, and 33, the Freedom of Information Act (FOIA) (5 U.S.C. 552), as amended, and the Privacy Act of 1974 (5 U.S.C. 552a), as amended and must be managed and scheduled for disposition only as permitted by statute or regulation.� In accordance with 36 CFR 1222.32, Contractor shall maintain all records created for Government use or created in the course of performing the contract and/or delivered to, or under the legal control of the Government and must be managed in accordance with Federal law. The Contractor shall not remove material from Government facilities or systems, or facilities or systems operated or maintained on the Government s behalf, without the express written permission of the Head of the Contracting Activity. When information, data, documentary material, records and/or equipment is no longer required, it shall be returned to [Agency] control or the Contractor must hold it until otherwise directed. Items returned to the Government shall be hand carried, mailed, emailed, or securely electronically transmitted to the Contracting Officer or address prescribed in the [contract vehicle]. Destruction of records is EXPRESSLY PROHIBITED unless in accordance with Paragraph (4). (5) Subcontracting. An SDVOSB subcontractor must meet the NAICS size standard assigned by the prime contractor and be certified and listed in the SBA certification database to count as similarly situated. Any work that a first tier SDVOSB subcontractor further subcontracts will count towards the percent of subcontract amount that cannot be exceeded. For supply or construction contracts, the cost of materials is excluded and not considered to be subcontracted. When a contract includes both services and supplies, the 50 percent limitation shall apply only to the portion of the contract with the preponderance of the expenditure upon which the assigned NAICS is based. For information and more specific requirements, refer to 13 CFR 125.6. (e) Required limitations on subcontracting compliance measurement period. An SDVOSB shall comply with the limitations on subcontracting as follows: [] By the end of the base term of the contract or order, and then by the end of each subsequent option period; or [] By the end of the performance period for each order issued under the contract. (f) Joint ventures. A joint venture may be considered eligible as an SDVOSB if the joint venture complies with the requirements in 13 CFR 128.402 and the managing joint venture partner makes the representations under paragraph (c) of this clause. A joint venture agrees that, in the performance of the contract, the applicable percentage specified in paragraph (d) of this clause will be performed by the aggregate of the joint venture participants. (g) Precedence. The VA Veterans First Contracting Program, as defined in VAAR 802.101, subpart 819.70, and this clause, takes precedence over any inconsistencies between the requirements of the SBA Veteran Small Business Certification Program and the VA Veterans First Contracting Program. (h) Misrepresentation.
