Description
Statement of Work Albany Stratton VAMC Emergency Repair Shimadzu Sonialvision G4 Planning UID: VA-21-00030423 Funding 2237: 528-21-2-8803-0129 Background: The Stratton VA Medical Center, 113 Holland Avenue, Albany, NY 12208 is currently seeking the emergent repair of a Shimadzu Sonialvision G4. Unit SN: 41C35165A001 located in the facility in room number A225-1-AL. The system has been removed from service due to patient safety concerns by our radiologist associated with collimator failure. The collimator cannot be repaired independently, and the entire collimator assembly must be replaced. Scope of Work: Contractor is required to furnish all parts and labor to accomplish the task of repairing the Shimadzu Sonialvision G4 unit to original OEM specification. All parts are required to be OEM parts to ensure proper functionality and reliability. Contractor shall test the functionality of the equipment upon completion of the repair. All testing shall be done to ensure unit is functioning per OEM specifications and units are fully operational per OEM guidelines as well as JACHO and FDA regulations governing medical equipment maintenance. Contractor will provide a detailed service report upon completion of the repair within 24hrs. Technician must be trained and certified by the OEM manufacturer to perform repair. Period of Performance Work shall begin immediately upon award unless otherwise specified. Work shall be completed within 5 days of award, unless a detailed explanation of delays has been provided to the VA personnel. Place of performance Stratton VA Medical Center, 113 Holland Avenue, Albany, NY 12208 in A225-1-AL. Inspection Repairs will be evaluated by the COR for this contract or other designated Healthcare Technology staff to verify repairs have been accomplished properly. No invoices will be processed for payment prior to the completion of this inspection. THIS REQUIREMENT IS AN EMERGENCY REPAIR FOR THE SHIMADZU SONIALVISION G4 UNIT TO ORIGINAL OEM SPECIFICATIONS.
