- This Request For Quote (RFQ) is Set-Aside for 100% Service-Disabled Veteran Owned Small Business (SDVOSB). Solicitation to be issued on or about June 5, 2020. Quotes will be due 25 days from issuance. Quotes are to be submitted via email no later than Friday, July 10, 2020 2PM CST. Quotes shall be sent via email to Sherry.Atkinson2@va.gov. Contractor shall provide all labor, management, personnel, equipment including secured vehicles, supplies and materials, transportation, applicable licenses, permits, and supervision, and all other related services necessary to provide on-site commercial document destruction services for confidential waste(administrative papers, sensitive documents, hard bound journals, magazines and white mixed paper, all temporary paper records which contain personally identifiable information (PII), personal health information (PHI), and VA sensitive information at the Central Arkansas Veterans Health Care System Medical Centers located at 4300 West 7th Street, Little Rock AR 72205 AND 2200 Fort Roots Drive, North Little Rock, AR 72114, campuses to encompass its divisions and three (3) offsite facilities located in Little Rock, Arkansas, and eight (8) Community Based Outpatient Clinics. Anticipated contract to be Base 12-month performance period with four (4) one-year options to be exercised at the discretion of the Government. Sensitive records are records that are national security classified or exempt from disclosure by statute, including Privacy Act, or regulation. Information protection, destruction precautions, the documents (paper data), CAVHCS information containing social security number and other information covered by the Privacy Act shall be destroyed. The NAICS code to be utilized for this acquisition will be 561990 Document Shredding and the small business size standard applicable is $11 mil Interested capable sources to provide: 1) Company name, address, point of contact, phone number, e-mail address, and DUNS number. 2) SAM registration and/or ORCA as evidence of size status; SDVOSB Certification. 3) Brief capability statement (no more than 5 pages) with enough information to clearly demonstrate the capability to provide the full range of required services, and information on past performance providing relevant services.
