The purpose of this Amendment is to issue the answerd of question submitted by several interested vendors. 2, Please refer to pages 2 and 3 of this Amendment for the answers of these questions. 3, Terms and Conditions of the Solicitation remain the same. Rafael Rodriguez Contracting Officer Page 3 of 3 1. Per Day, what is the frequency of pick up between St. Croix and St. Thomas at the facilities where specimen will be picked up? Does it fluctuate or change daily? One pick-up on Tuesdays and Wednesdays for St. Thomas and One Pick-up on Wednesdays and Thursdays for St. Croix. It doesn t fluctuate or change on daily basis. 2. What is the approximate daily weight of specimen needing transport�to VA San Juan? �2 boxes approximately 15 pounds on average (together). 3. For transportation purposes between St. Croix/St. Thomas to VA San Juan, PR the use of an aircraft would be a part of our quote.� Is this acceptable? � Transportation is currently via airplane. � 4. If aircraft for transport is acceptable, is it limited to a specific�type of aircraft? Any specific aircraft certifications needed? As far as pilot licensure is concerned, what is the requirement? Private pilot, commercial pilot? We will be sending packages with EXEMPT patient samples according to IATA 3.6.2.7: Specimen taken directly from a patient that have minimal likelihood of containing a pathogen. No history of infections. No disease symptoms and not related to local disease outbreaks. � 5. Shipping material that most also be provided, do you have approximately an estimation�of the daily need? Dimensions�and or weight minimum and/or maximum for the specimen? Card Boxes with insulated (foam) material approximately 1 cubic feet. Daily need: Two boxes / per clinic / per shipping 6. Is the daily transport of these specimen from one place to the other limited to immediate delivery to VA San Juan, PR or can 2 or more flights/trips be scheduled for more productivity (morning and afternoon) -Same with locally picking up specimen from the clinics at St.Croix/St. Thomas, is it set schedule? will customer call between the business hours of M-F 8:00a to 4:40p for multiple pick ups? or can this be streamed line for productivity and cost effectiveness? The sample viability is approximately 24 hours. The clinic (St. Thomas and St. Croix) is open until 2:00pm. The delivery at VA San Juan Medical Center can occurred 24-7. 7. What is the total number of pick up locations of specimen? Pick-ups are locations at two (2) locations: VA Community Health Clinics at St. Thomas and St. Croix When the Price/Cost Schedule attachment of this described the service as 288 deliveries . The 288 deliveries are: 288 samples, 288 tests or 288 boxes? There are approximately 4 deliveries per week. Each delivery consists of two boxes with multiple samples. The boxes will be ready at location with samples inside. The amount of 288 deliveries are expected to be handle out on a daily, weekly or on a monthly basis? Each delivery expected to be handled on Tuesdays and Wednesdays from St. Thomas and Wednesdays and Thursdays from St. Croix Considering that blood samples have different requirements to be transported per example: Frozen sample, refrigerated samples, centrifuge samples etc. What are the specific types of blood samples that be handle? The sample are going to be handle by VA Laboratory personnel. Usually two boxes are shipped. One box with ice. � Are the sample data going to be managed by the Direct Data system? Sample labelling is performed by VA personnel. The samples will be placed in boxes at the time of pick-up. Are the contractor supposed to buy new boxes or VA will be providing the boxes? The contractor will provide the boxes. On the solicitation the term quantities expressed on the Price Cost Schedule refers to Pounds, boxes, samples?� It refers to service provided on a monthly basis (service per month). What type of packaging is required? ������ � Foam Boxes covered with cardboard maintaining the �temperature required for the samples. What is the Temp requirements for the Pkgs?�� Refrigerated samples- 2�-10�C� What are the Pickup times for Both St Thomas and St Croix? � � �1pm in the clinic front desk Important note: samples will be received the same day in� VACHS San Juan main lab.
