AMENDMENT 01 ***SEE ATTACHED AMENDMENT 01 DOCUMENT FOR FULL DETAILS*** The purpose of this amendment is to correct the Cutter�s homeport FROM: Mobile, AL TO: Newport, RI on page 11-12 of 54 of the Solicitation under �FORESEEABLE COST FACTORS PERTAINING TO DIFFERENT SHIPYARD LOCATIONS� items (1) and (3)(iv). Attachment �IDA LEWIS DD FY20 Solicitation� is replaced with �IDA LEWIS DD FY20 Solicitation Rev1� Vendors do not have to wait until 17 February 2020 to submit its quote and can submit its quote any day or time prior to 17 February 2020 if it prefers.� Also vendors should be aware that sometimes there are issues in electronic submission of quotes and it takes longer than expected for the quote to arrive in the Government email(s).� Since quotes must be received NOT LATER THAN 17 February 2020 at 11:30 AM PT, it is best not to wait until the last minute to email the quote/proposal. All other terms and conditions of the combined synopsis/solicitation remain unchanged. ***SEE ATTACHED AMENDMENT 01 DOCUMENT FOR FULL DETAILS*** ORIGINAL SOLICITATION ***SEE ATTACHED RFQ DOCUMENT FOR FULL DETAILS*** This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart12.6, supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a separate written solicitation will not be issued. The solicitation is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC�2020-04�Effective 15�January 2020. This requirement is being solicited as a total small business set aside using FAR Part 12 Acquisition of Commercial Items and FAR 13.5 Simplified Procedures for Certain Commercial Items. The NAICS Code is 336611 � Ship Repair with a small business size standard of 1,250 employees. The solicitation number is 70Z085-20-Q-P4529800. SCHEDULE OF SUPPLIES/SERVICES: See Attachment 1. DESCRIPTION OF WORK: The Contractor shall furnish all necessary labor, materials, services, equipment, supplies, power, accessories, special tools and such other things as are necessary to perform various Dry Dock Repairs of the USCGC IDA LEWIS (WLM 551), a 175 foot Buoy Tender, all in exact accordance with this solicitation and the attached specification. See Attachment 2. Scope of services shall include ship repair and associated support work, and may include work that is not currently listed as Definite Items. CONTRACT TYPE:� This solicitation is a request for quotes (RFQ) that will result in the award of a firm fixed price contract solicited under FAR PART 12 and FAR Subpart 13.5 Simplified Procedures for Certain Commercial Items, subject to availability of funds. CONTRACT PERIOD OF PERFORMANCE: The performance period for this requirement is 23 April 2020 through 02 July 2020. PLACE OF PERFORMANCE: The place of performance is the Contractor�s Facilities. The cutter is not geographically restricted. CUTTER�S HOMEPORT:� 47 Chandler Street, NAVSTA NETC, Pier 2, Newport, RI 02841-1718 REQUIRED DOCUMENTS: Each quoter shall furnish the information required by the solicitation, which includes: Cover letter with quoter�s address, DUNS and other information completed. Amendments, as applicable Attachment 1- Section B Schedule of Supplies/ Services filled out with a cost associated with each CLIN, to include option labor hours. The attached excel spreadsheet should be filled out with prices and returned with the vendor�s quote. Attachment 3 - Past Performance Documentation Small Business Self Certification for applicable set-aside (i.e. HUBZone) Welding Certifications and Procedures Proof of Insurance (Statement of Coverage from current Insurance carrier) List the names of ALL Subcontractors that your company intends to subcontract for the CLIN Item(s) listed in the Schedule of Supplies/Services. If applicable, ALL Technical Representative Certifications for BOTH prime and subcontractors as specified in the specification.
