- This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation 36C25020Q0083 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2019-4 (eff. 10/10/19), VAAR update 2008-19 (eff. 10/04/19) & Acq. Policy Flash 20-01 (eff. 10/04/19). The North American Industry Classification System (NAICS) number is 325998 and the business size standard is 500 employees. ************************************************************** REQUIREMENT: The Contractor will provide and deliver Potassium/Magnesium Mix pellets and Rock Salt in bulk per ton as requested by the Grounds and Transportation Services of the VA Ann Arbor Healthcare System. Deliveries of the potassium/magnesium mix pellets and rock salt must be received within 48-hours of telephone order from the Grounds and Transportation services representatives. Telephone requests will be in 10 ton increments per product. Estimated quantities are as follows: Potassium/magnesium mix pellets: 100 tons Rock salt: 250 tons ************************************************************** The proposed contract listed here is SET-ASIDE for Small Business vendors (SB) only. FOB destination. Deliver to: VA Ann Abor Healthcare System 2215 Fuller Rd. Building 1 Ann Arbor, MI 48105-2303 Offerors must comply with all instructions contained in provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. 52.204-7 and 52.204-16. 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation. Award will be based on lowest price technically acceptable offer (LPTA). 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer unless SAM registered which must be noted in the quotation. 52.212-4, Contract Terms and Conditions-Commercial Items.
