Page 2 of 10 This is a Sources Sought Notice (a) The Government does not intend to award a contract on the basis of this Sources Sought or to otherwise pay for the information solicited. (b) Although "proposal," "offeror," contractor, and "offeror" may be used in this sources sought notice, any response will be treated as information only. It shall not be used as a proposal. (c) Any information received from a contractor in response to this Sources Sought may be used in creating a solicitation. Any information received which is marked with a statement, such as "proprietary" or "confidential," intended to restrict distribution will not be distributed outside of the Government, except as required by law. (d) This Sources Sought is issued by VISN 2 Contracting Office for VISN 2, which includes of collecting information about Bruan AquaBoss Dialysis Equipment Service Agreement at the New Jersey Healthcare Systems, East Orange, 3385 Tremont Avenue, East Orange, NJ 07018. The VA is seeking to provide for this requirement for October 1st, 2019. The NAICS code identified for this requirement is 811219-Other Electronic and Prevision Equipment Repair and Maintenance The requirement is detailed in the Statement of Work section of this document. (e) Cursory market research has identified one large business that may be able to provide for this requirement. Contractors that are able to meet the requirements in the attachment are encouraged to email a capability statement and full information to Megan Ross at Megan.Ross3@va.gov. - Part of the purpose of this sources sought is to determine the viability of set aside to a specific socio-economic category. Contractors shall identify the NAICS code for the services being offered as well as their size status under said NAICS. This information will be used to determine the viability of a set aside for this requirement. - Contractors shall also identify any Federal Supply Schedules that may carry the desired services. - Contractors shall identify pertinent point of contact for company, contractor DUNS number for size standard and socioeconomic verification in SAM and VIP. - Contractors shall include any relevant comments about the Attachment(s) if applicable. 9 Page 9 of 3 SCOPE OF WORK SUMMARY: The Biomedical Engineering department at VA New Jersey Health Care System is requesting a five (5) year (Base + four) Full Service contract covering the following dialysis equipment: Manufacturer Product Code Serial/ Batch Number Braun AquaBoss LA1107107 Eco Dia II 1050 HT 2000000529 Bruan LA2050421 11000000233 Bruan K-A-PRE-TX-1 Place of Performance/Place of Delivery: New Jersey Healthcare Systems, East Orange 385 Tremont Avenue East Orange, NJ 07018 Period of Performance: Base Year: 10/01/2019-09/30/2020 Option Year 1: 10/01/2020-09/30/2021 Option Year 2: 10/01/2021-09/30/2022 Option Year 3: 10/01/2022-09/30/2023 Option Year 4: 10/01/2023-09/30/2024 Scope or Tasks (for services): The company will perform water sample testing as well as service all of the equipment above according to manufacturer specifications. Both the water sample testing and the service of equipment will take place every month through the covered term. Failed parts under normal operating usage should also be covered under the contract as a part of convenience. All data and device settings will be optimized. Alarm Data Log book shall be analyzed for errors and product defects. All parts and equipment shall be inspected, and micron all five (5) filters replaced. Equipment must meet all CDC, NYS, and HHS requirements. Qualification: Vendor must be certified by the original equipment manufacturer (OEM) to provide service repairs on the above listed equipment.
