- Attachment A - Price Schedule Attachment B - Synopsis-Solicitation RFQ1381281 Attachment E - Tax Exemption Form Attachment D - Provisions Attachment C - Clause Admin Data This solicitation is set-aside for Service Disable Veteran Owned Businesses and is a Brand Name or Equal. This solicitation is issued under Request for Quotations (RFQ) and the solicitation number is RFQ1381281. This acquisition is a Service Disabled Veteran Own Business Set-Aside. The North American Industry Classification System (NAICS) code is 339112 and the size standard is 1,000 employees. The White River Service Unit (WRSU) is seeking qualified vendors who can provide a Point-of-Care Portable Ultrasound Equipment with Accessories for the WRSU. Offers must meet the requirements set forth within the Request for Quote (RFQ) and must be capable of providing a Fujifilm Sonosite M-Turbo Ultrasound System with Accessories or a product that is Equal by meeting the Salient Characteristics detailed in the RFQ. The Government anticipates an award of a Firm Fixed Price Purchase Order. The Government intends to award one Purchase Order as a result of this solicitation. The solicitation will be released on 09/17/2019 and will be distributed solely through the General Services Administration's Federal Business Opportunities website at www.fbo.gov. Hard copies of the solicitation will not be available. The website provides all downloading instructions to obtain a copy of the solicitation. The Line Items are listed on Attachment A - Price Schedule. Offers shall enter all prices for all the Line Items on the Attachment A - Price Schedule. Not providing prices for all Line Items shall make your offer non-responsive to the solicitation. Offerors shall provide brief description of alike salient characteristics of the items stated in the Schedule, to include technical specification and product literature documentation to support products being offered as equal items. The Offerors are required to submit RFQ by email to: bradley.platero@ihs.gov. The RFQ due date is close of business 12:00 pm Phoenix, Arizona Time on September 20, 2019. Late proposals will not be accepted. All contractors doing business with the Federal Government shall be registered in the System for Award Management (SAM) database. The website for registration is www.sam.gov. It is the Offerors responsibility to periodically check for any amendments to this solicitation.
