Description
(i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This Request for Quotation (RFQ) is being issued via solicitation number 75H70719Q00058. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisitions Circular 2005-98. (iv) Pursuant to the authority under FAR 19.5. This requirement is a Small Business Set-Aside and only qualified Offerors may submit bids. The associated North American Industrial Classification System (NAICS) code for this procurement is 335312. (v) The Contract Line Items (CLIN) and pricing structure are as shown below: Generator Preventative Maintenance Services (vi) Fee Schedule: Generator Preventative Maintenance Services will be purchased. SEE ATTACHMENT (Fee Schedule) (vii) The purpose of this fixed price purchase is to procure Generator Preventative Maintenance Services with the following Scope of Work: GENERAL A. Services: The purpose of this contract is to provide Generator Preventative Maintenance specialized testing, inspection services and repair work to ensure that the AAIHS facilities/buildings are in compliance with National Fire Protection Association (NFPA) and The Joint Commission (TJC) accreditation requirements regarding the Emergency Generator. The Contractor for this work shall provide all material, equipment, labor, tools, etc., to perform Generator Preventative Maintenance Services as described and at the facilities located within the Zuni Service Unit, Albuquerque Area Indian Health Service (AAIHS). B. Location of Work: The work is to be performed at the Zuni Hospital, Maintenance facilities described in this SOW. C. Minimum Contractors Qualifications: The Contractor shall meet the minimum qualifications and possess the required experience as follows: 1. Have a minimum of five (5) years' experience in the Generator Preventative Maintenance Services and in the work of inspecting systems for compliance with code requirements. 2. Have performed similar type projects in scope and size within the last two (2) years. 3. Have experience in the New Mexico/Arizona/Texas/Colorado geographical area. 4. Have the ability, including equipment and personnel, to provide reports required elsewhere in this Scope of Work (SOW) within 30 calendar days after completion of work. D. Facility Overview and Procedures: 1. The Contractor shall attend and participate in an initial meeting with the Contracting Officer's Representative (COR), Supervisory Maintenance Mechanic and Maintenance staff from the date of award from the Contracting Office. The pre-performance meeting agenda shall include, at a minimum, a review of the specific facility's Contractor procedures, safety and security measures, work schedule, and any required coordination. 2. The Contractor shall provide a written report of the schedule work to be performed for the Service Unit at the pre-performance meeting. These documents shall be reviewed to ensure that the reporting periods meet all facility and code requirements. 3. The Contractor shall perform all work during normal business hours, 8:00 a.m. to 4:30 p.m., Monday through Friday, excluding Federal Holidays. These service will be done on a need base or annually with Authorization other than normal work hours shall be coordinated and approved in advance with the Contracting Officer's Representative/Facility Supervisor. 4. The scheduled work shall be coordinated with the respective Contracting Officer's Representative/Facility Supervisor. The inspections and all work are to be completed with the facilities remain in operation. The Contractor shall ensure that the services don't disrupt patients care. 5. The Contractor shall sign in and out with the Facility Maintenance office during every visit. A Contractor's badge shall be provided to each crew member on-site and shall be worn as work is performed. Badges shall be returned at the end of the work day or at the end of the service period as determined by the Facility/Project Manager. 6. The Contractor shall adhere to all policies, standards, and requirements of the respective facility while on-site. Safety procedures shall be strictly adhered to at all times. PROJECT DESCRIPTION: The contractor shall provide a one-time 36 months' load testing for four (4) hours on each generator. Preventive Maintenance Services (PM), Maintenance and Emergency Repairs on Emergency Generators for the Zuni facility located at Hwy RT. 301 North 53 B- Street P.O. Box 467 Zuni New, Mexico 87327. A. Zuni Hospital - Emergency Generator a) Change oil and oil filter(s). b) Inspect contents of oil filter for debris. c) Draw oil sample and provide customer with analysis report. d) Inspect all engine safety circuits and control systems tested for proper operations. e) Engine coolants tested and serviced; inspect all belts and hoses; tighten hose clamps as needed. f) Inspect engine for leaks and/or component wear; verify all fluid levels. g) Inspect complete fuel delivery system, inspect and grease governor linkages. h) Inspect battery charger and test batteries for maximum reliability. Record voltage levels during crank cycle and establish minimums and trends. i) Generator auto-control system inspection: 1) Check all wiring for wear and/or damage. 2) Verify operation of indicator lights. 3) Itemize condition and any needed repairs, if required. 4) Check all timers for correct values; adjust if required. j) Operate generator at no-load: 1) Check engine pressure and temperature values. 2) Check generator voltages and frequencies; voltage regulation, controls, and safety devices for proper operation. k) Shall dispose of old crankcase oil and materials used during the service. j). Provide a detailed report of work performed and any recommendation(s) to maintain a high level of reliability. m) Miscellaneous parts and additional cost for repairs and replacements. B. Maintenance Repairs: a. Contractor will shut down generator and secure remote switch before performing work. b. Work will entail removing coolant from unit and save to reuse, remove hoses and piping. c. Dismantle radiator fan housing to remove fan, alternator belts. d. Inspect all moving parts for leaks, excessive wear. e. Inspect cooling coils for leaks or damage. f. Pressure wash or steam clean the cooling coils of debris. g. All parts replace and housing reassemble, contractor will test for leaks. h. It is the responsibility of the contractor to clean up waste and materials for proper disposal. i. If any parts or materials needed to be replaced immediately; a separate invoice will be initiated for approval. C. Load Test for the Emergency Generators: a. Contractor shall perform load bank test at 75%, 4 hours' load bank test for the emergency and alternate generator: 1. Load test using portable load bank provided by the contractor. Tests run the generator for a minimum of 4 hours at 75% rating. A load test report shall be used to log all data during the load test. 2. Check and record generator voltage and frequencies; voltage regulation controls and safety devices for proper operation. 3. Contractor shall provide a detailed report of work performed and any recommendations to assure maximum reliability. D. Emergency Repairs for the Emergency Generator: a. Provide up to 2 emergency services call per year should the emergency or backup generator experiences mechanical/electrical failures. b. Contractor will also provide portable generator for backup system to prevent any down time while generator is being repaired. c. Troubleshoot and diagnose problems reported and corrected. d. Repair systems as required by HHCC. e. Test all components after repair for reliability. f. Check generator for safety shutdown components for proper operation. g. Miscellaneous parts and additional cost for repairs and replacements must be approved prior to doing the work. h. Any additional modification or fabrication to increase productivity and dependability as recommended by service contractor shall be approved by the Facility Engineer. WAGE DETERMINATIONS UNDER THE SERVICE CONTRACT ACT Wage Determinations under the Service Contract Act by Direction of the Secretary of Labor are hereby incorporated under WD 15-5451 (REV-7) from the www.wdol.gov on 01/01/2019. PERIOD OF PERFORMANCE: Period of performance will be from TBD, 2019 through TBD, 2020 with four one (1) year options for renewal.
