THIS REQUEST FOR INFORMATION (RFI) 3626219Q0501 -IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a source sought/RFI only. The purpose of this source sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 339112 (size standard of 1,000 Employees). Responses to this source sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this source sought, a solicitation announcement may be published. Responses to this source sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office, is seeking sources that can provide Patient Lights for the new Emergency Room that at a minimum meets the following salient characteristics with brand name for the VA Loma Linda Healthcare System: LINE ITEM DESCRIPTION QUANTITY UNIT UNIT PRICE AMOUNT 0001 Mira LED 90, Ceiling Mount, Double, Standard Arm. Stock # L-MLED90-CM-DC-ST 2 EA   $ - 0002 Mira LED 65, Wall Mount, Standard Arm. Stock # L-MLED65-WM-XX-ST 12 EA   $ - TOTAL $ - Include shipping cost on the items price. Vendor Requirements: Vendor shall provide delivery after receipt of order. Vendor shall provide manufacture standard warranty on the product starting from the first day of delivery. STATEMENT OF WORK FOR THE VA LOMA LINDA HEALTHCARE SYSTEM FOR THE PATIENT LIGHT FOR THE EMERGENCY ROOM SCOPE: The VA Loma Linda Healthcare System has a requirement to deliver patient lights within the new activated Emergency Room (ER). The exam lights provided will be servicing the new emergency department. Installation is not included. The ceiling mounted double arm Mira LED 90 will be located in the Life Support Rooms and the wall mounted single arm Mira LED 65 will be located in the Exam Rooms (See the attached partial reflected ceiling plan for reference). PRODUCT DESCRIPTION: LINE ITEM DESCRIPTION QUANTITY UNIT 0001 Mira LED 90, Ceiling Mount, Double, Standard Arm. Stock # L-MLED90-CM-DC-ST 2 EA 0002 Mira LED 65, Wall Mount, Standard Arm. Stock # L-MLED65-WM-XX-ST 12 EA SALIENT CHARACTERISTICS These exam lights shall meet the specific salient characteristic to be compatible with the current design and build out of the ER in order to provide ample space and quality service during an emergency situation. The minimum Salient Characteristics requirements listed below shall be met as follows: A minor surgery and exam/procedure light. Light Intensity (Central Luminance) varying outputs: 65,000 Lux / 90,000 Lux Brightness Control: 50% - 100% Light Field Diameter: 6.5" to 12" (measured at 1 meter) Endoscopy mode Depth of Illumination: 94"(measured at 1 meter) Color Rendering Index: 95 Shadow Dilutions (approximate): 0.7% for Single Mask 55.5% for Double Mask 100% for Cavity 0.3% for Single Mask with cavity 55.5% for Double Mask with cavity Number of LED's (main): 36 (leads to superior shadow control) Total radiant power at max intensity: approximately 231.4 w/m2 Floor stand Approximate Dimensions: Floor stand: 26" L x 25" W x 65" H Procedure Light arm: 40" L Interchangeable light heads (future proofing & acuity adaptable) INSTALLATION Installation is not required for this requirement. PLACE OF PERFORMANCES: VA Loma Linda Healthcare System 11201 Benton Street Warehouse Building 1C05 Loma Linda, CA 92357 CONTRACT TYPE: This will be a Firm Fixed Price contract. PERIOD OF PERFORMANCE: 30 Days after receipt of order. DELIVERY Vendor shall deliver all supplies 30 days ARO. WARRANTY All supplies shall have standard warranty starting from the first day of delivery. NORMAL WORKING HOURS: Monday Friday, 8:00AM 4:00PM (Pacific Time) Federal Holidays observed The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. If you do, state how and what is altered; assembled; modified? (5) Does your company have an FSS contract with GSA or the NAC or are you a contract holder with NASA SEWP or any other federal contract? If so, please provide the contract type and number. (6) If you are an FSS GSA/NAC or NASA SEWP contract holder or other federal contract holder, are the referenced items/solutions available on your schedule/contract? (7) Please provide general pricing for your products/solutions for market research purposes.
