Sources Sought Notice Indiana County - Market Research Notice: This advertisement is for market research purposes for a potential opportunity. This advertisement is not a solicitation for offers, nor is it a request for proposals. The purpose of this advertisement is to identify potential sources and suitable locations and is not intended to pre-qualify or disqualify any potential offers. The Government will not pay for any costs incurred because of this advertisement. The Government cannot provide any warranty, expressed or implied, as to the accuracy, reliability or completeness of contents of the furnished information found within this advertisement. Respondents are advised that the Government assumes no responsibility to award a lease based upon responses to this advertisement The Altoona VA Medical Center, Altoona, PA desires to identify potential sources that have the capability to provide up to 11,386 Net Usable Square Feet (NUSF) of leased space for a Community Based Outpatient Clinic within the delineated area identified below. Space requirement includes a contiguous space (preferable on one floor but not more than two floors) with ADA (Americans with Disabilities Act) accessibility and VA Barrier-Free Design Standard and Architectural Barriers Act Accessibility Standards, and have 40 parking space, to include 4 ADA spaces or more if required by local law or statute. Space must accommodate PACT (Patient Aligned Care Team) space model (http://www.cfm.va.gov/til/Prototype/CBOCPrototype-02-ProjNarrative.pdf ). The anticipated lease term is a firm term of five (5) years, and additional soft term of ten (10) years for a total lease term of 15 years. The space must be in or abounding the delineated area. Delineated Area: Indiana County, PA South:   At the corner of State Route 22 heading North on 217, 217 North turns into 286 North East to State Route 422 North West. Continue on State Route 422 to 5 Points Road North. West:   5 Points Road North to Creek Side Road East, to 954/Shelocta Street East to the Town of Creekside, PA.   North: Turn onto Route 110 East and South through the Town of Ernest, PA. Continue on Route 110 East/South to Lutz School Road (SR 1003). South East State Route 1003 merges back onto Route 286 East from 286 East (listed as Clymer Road). East:   286/Clymer Road turns into Airport Road.   Airport Road West onto State Route 119 South.   Take 119 South through Homer City and Blacklick onto State Route 22 West. Intersection of 119 South and State Route 22 West to Route 217.   Space will not be within a 100-year flood plain. Space within the 100-year flood plain would only be considered if no space was available outside the 100-year flood plain. If applicable, historical property should be identified for consideration. If applicable, socioeconomic classification should be identified for consideration. Interested parties should submit information sufficient to show space fulfills the basic requirements stated in this source sought notice: outside 100-year flood plain; historical or non-historical space identification; within delineated area; meets the requirement of amount of space and ability to provide PACT design. Responses to this notice will assist VA in determining whether this acquisition should be set-aside for competition restricted to SDVOSB or VOSB concerns in accordance with 38 USC Sec. 8127 or proceed with Full and Open competition from capable sources. PLEASE NOTE: Although this notice contains detailed information for interested SDVOSB and VOSB parties, VA encourages all small businesses and other interested open market sources to respond for market research purposes. SDVOSB and VOSB firms are invited to provide information to contribute to the market research for this project. SDVOSB and VOSB firms must be registered in VA s Vendor Information Pages (VIP) site at https://www.vip.vetbiz.gov/ and have the technical skills and financial capabilities necessary to perform the stated requirements. Only SDVOSB and VOSB firms are requested to submit a Capabilities Statement to VA for review. SDVOSB and VOSB firms will provide a Capabilities Statement if they are interested in participating in this project. Capabilities Statement Will Include: 1. A summary describing up to three (3) projects of similar size and scope completed in the past five (5) years that demonstrate your company s experience designing, constructing, and managing Federal leased facilities or health care facilities relevant to the project described above. (3-page limit); 5. Evidence of capability to obtain financing for a project of this size. If desired, the company may also submit a narrative describing its capability. Although this notice focuses on SDVOSB and VOSB, we encourage all small businesses and other interested parties to respond for market research purposes. This is not a solicitation for offer, but an inquiry for outpatient clinic space.
