- THIS REQUEST FOR INFORMATION (RFI) 3626219Q0492 -IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a source sought/RFI only. The purpose of this source sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 339112 (size standard of 1,000 Employees). Responses to this source sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this source sought, a solicitation announcement may be published. Responses to this source sought synopsis are not considered adequate responses for a solicitation announcement. Vendor Requirements: Vendor shall provide delivery after receipt of order on September 2, 2019. Vendor shall provide manufacture standard warranty on the product starting from the first day of delivery. STATEMENT OF WORK FOR THE VA LOMA LINDA HEALTHCARE SYSTEM FOR THE EYE CLINIC LIGHTS SCOPE: The VA Loma Linda Healthcare System has a requirement to deliver and install EDS lights suspension. Stock # 0682001574A 1 EA 0006 Installation BNL-Light/Flat Panel Suspension 1 JB 0007 Installation -Implementation Staging Charge 1 JB 0008 ICTS Engineering 1 JB CONTRACTOR RESPONSIBLITES: Coordinate with hospital personnel for a proposed time frame for installation of Stryker-supplied infrastructure Check in with hospital personnel to announce arrival Engineering services to include professional project engineer/project manager, CAD team, and all necessary oversite for installation of ceiling suspension Provide all required shipping for equipment and parts. Provide installation labor to receive, inventory, and transport parts/equipment to Eye Clinic building from receiving warehouse on VA Loma Linda campus. Provide labor to install all light components below the ceiling. Provide the hospital with CAD drawings including elevation and room configuration drawings for Stryker-supplied equipment INSTALLATION Vendor shall install the surgical lights and calibrate the brakes according to Stryker specifications. Vendor shall connect low voltage electrical and data cables for surgical lights and adjust lighting (voltage, field diameter adjustments, etc.) to Stryker specifications. Vendor shall install the flat panel monitor arm and calibrate the brakes to Stryker specifications. Vendor shall route and terminate all audio-visual cables required for Stryker-supplied equipment Vendor shall install flat panel monitor and connect audio visual wiring kit to the flat panel PLACE OF PERFORMANCES: VA Loma Linda Healthcare System 11201 Benton St. Loma Linda, CA 92357 CONTRACT TYPE: This will be a Firm Fixed Price contract. PERIOD OF PERFORMANCE: September 2, 2019 January 30, 2020 DELIVERY Vendor shall deliver all supplies on September 2, 2019 WARRANTY All supplies shall have standard warranty starting from the first day of delivery.
