9 THIS IS A PRE-SOLICIATION NOTICE The Department of Veterans Affairs, Murfreesboro TN is preparing to issue a Request for Information for (RFQ) for: Testing, Service and Maintenance of Fire Equipment at James H. Quillen VA Medical Center, Mountain Home, TN. SET-ASIDE: 100% Service-Disabled Veteran-Owned Small Business (SDVOSB) NAICS: 561621 Security Systems Services SMALL BUSINESS SIZE STANDARD: $20.5 million Type of Contract: Firm Fixed Price (FFP), and a one-year base period, with four one-year option periods. PROJECT OVERVIEW: TESTING, INSPECTION AND MAINTENANCE OF FIRE ALARM SYSTEM SCOPE The Mountain Home Veteran Affairs Medical Center (VAMC) requires annual inspection and testing of all fire alarm systems located on the installation for the period of: Base Year: 4/15/2019 - 9/30/2019 Option Year 1: 10/1/2019 - 9/30/2020 Option Year 2: 10/1/2020 9/30/2021 Option Year 3: 10/1/2021 9/30/2022 Option Year 4: 10/1/2022 9/30/2023 PERSONNEL AND EQUIPMENT: a. The contractor shall maintain qualified personnel, supervision, and equipment to accomplish work on schedule, to the satisfaction; as specified under this contract. b. Employees and supervisors shall be capable and experienced in performing fire alarm maintenance, tests, and inspections as specified in the contract. The servicing contractor shall be certified by the State of Tennessee Department of Commerce and Insured as an Alarm Systems Contractor. Additionally, all service personnel of the contractor must have been certified as an Alarm System Technician under the State of Tennessee License. c. The contractor shall provide evidence of State of Tennessee Board for Licensing Contractors for Contractors and Alarm Systems Contractors Board. The contractor s employees performing the work shall be certified by the National Institute for Certification in Engineering Technologies, NICET in Fire Alarm Systems and Inspection and Testing of Fire Alarm Systems and shall provide evidence of certification. The supervisor of the employees providing inspection, testing, and maintenance service, shall be certified NICET Level III in Fire Alarm Systems. The supervisor is required to oversee the work performed and be on site monthly to discuss the required monthly reports. The person performing the inspection and testing of the fire alarm systems shall be NICET Level II certified in Inspection and Testing of Fire Alarm Systems. The person performing maintenance, programming, and installation services shall be NICET Level III certified in Fire Alarm Systems. d. The fire alarm panels on Mountain Home s campus are Simplex Fire Alarm Panels. The contractor shall have certification and formal training from Simplex for Simplex 4010, 4010ES, 4100ES, 4100U, 4100+ and Network systems. e. Evidence of all required licenses, as well as documentation of the qualifications of personnel, shall be provided with your proposal. SPECIFIC TASKS: - The Contractor shall furnish all labor, supervision, tools, material, equipment, transportation, lift equipment and management necessary to provide testing/inspection, and preventive maintenance services for the existing fire alarm systems at the Department of Veterans Affairs Medical Center at Mountain Home, TN. Maintenance services may require installing new devices or moving devices etc. EXTENT OF WORK: a. There are also 3 network Display Units, NDUs that require an annual inspection: Building # 204 Simplex 4100ES NDU panel Building # 37 Simplex 4100U NDU panel Building # 2 (repeater), Simplex 4100U NDU panel c. There are four FACPs on the VA s fire alarm network that are not required to be inspected and tested annually, but they may require network maintenance and service. These buildings are leased facilities and the tenant is responsible for maintaining these FACPs. Building # 53 Simplex 4010 panel Building # 108 Simplex 4010 panel Building # 117 Simplex 4010 panel Building # 208 Simplex 4100+ panel d. The FACP s are located throughout the campus in 35 buildings which provide service for 39 buildings. FACP s are connected on a token ring fiber optic network. A log of all events, troubles, and alarms are maintained at the Campus Dispatcher s Office in Building #204 on a 4100ES NDU and at building 37 s Safety Office on a 4100U NDU. e. An alarm can be initiated by any of the following types of actuators: - Manual Pull Stations (key operated and pull-down types), Automatic Smoke Detectors (photoelectric and Ionization type), Automatic Heat Detectors, Automatic Duct Smoke Detectors (supervisory and alarm points) and Sprinkler monitor devices (supervisory and alarm points). f. Sprinkler water flow alarms need to be electronically tested in a timed mode to prevent false alarms caused by fluctuations in water pressure. The Flow Switches are to be tested and re-timed for a 30-second retard delay. All sprinkler tamper and supervisory points shall be electronically tested to verify they report to the FACP. g. This contract shall include a Central Station Services monitoring contract for all the fire alarm panels listed on this contract. The central station monitoring provider shall be listed and approved by the VA. Alarm signals, telephone lines troubles, and communication troubles from the VA to Central Monitoring Center are the only signals to be transmitted to the central station monitoring. The monitoring services will be accomplished using one dialer/communicator for all FACPs and it will be in the VA Police Dispatch Office NDU panel. Each VA building shall have its own monitor point that transmits alarm signals to central station monitoring. V. HOURS OF WORK: The contractor shall perform the inspections for the following buildings in the months indicated below. The inspections shall be scheduled on the 15th of every month with a tolerance of 5 business days before or after the 15th. January Building # 162 February Building #s 205 and 207 March Building #s 8, 36, 116 and 37 April Building #s 34 and 206 May Building # 204 June Building #s 3 and 5 July Building # 200 August Building #s 69 and 77 September Building #s 160 and 161 October Building #s 13, 15, 17, 19, 20 and 35 November Building #s 40, 41, 42, 43, 44, 45, 46 and 47 December Building #s 39, 115 and Parking Garage b. The contract shall perform periodic inspection, testing, and maintenance on Monday through Friday between the hours of 8:00 am and 4:00 PM, except on Federal Holidays listed below. Approval to perform periodic maintenance testing and inspection on other than specified times must be approved by the Safety Office. Audio devices, HVAC shutdown and elevator recall functions may be required to be tested after 4:30pm at Safety s discretion. The Federal Holidays are as follows: New Years Day Martin Luther King Jr. s Birthday Washington s Birthday (President s day) Memorial Day Fourth of July (Independence Day) Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day Contractor shall not be permitted to leave any fire alarm system or components of these systems out of service after 4:00pm without authorization from the Safety Office. The contractor may be required to perform maintenance services after hours (between the hours of 4:00pm and 8:00am), on weekends and holidays in emergency situations. The contractor shall respond to service requests with a phone call within 1 hour of the service request and arrive on-site within 24 hours of the service request. A maximum of 30 minutes of travel time can only be billed to the service hours portion of this contract for each service call. VI. PERIODIC INSPECTION, TEST AND MAINTENANCE: - All periodic inspections, tests and maintenance shall be performed in accordance with NFPA 72, VA requirements, and The Joint Commission criteria. Visual inspection of all fire alarm systems and their initiating devices, control panels and batteries for corrosion, voltage and proper wire termination. Visual inspection shall insure that all devices are properly mounted and shall include inspection of the structure/layout to ensure that furniture/and or structure does not compromise the function of the device. Alarm activation to Central Monitoring and Police Dispatch Office NDU shall be tested monthly for the respective building scheduled according to this scope of work. The fire alarm transmission to Central Dispatch shall be documented on the report with the time and date of this test listed. All alarm, supervisory and trouble signal transmission shall be verified at the VA s Police Dispatch Office. The contractor shall be responsible for doing any necessary system shutdowns and restoring system to working condition when services are performed. The contractor is responsible for notifying the Safety Office of any shutdowns on the system and when the system is placed back into service. The contractor shall provide personnel to perform a fire watch if it is necessary due to the shutdown. Operational tests for an alarm condition for each initiating zone circuit and addressable device: 1. Perform functional test of 100% of the initiating devices in each zone circuit for the building being tested (Initiating devices include smoke detectors, heat detectors and duct detectors). All addressable initiating devices must also be tested. This includes sensitivity testing for all smoke detectors. 2. Perform functional test of 100% of manual pull stations in each building being tested. 3. Perform functional test of 100% of water flow and supervisory monitor points for each building being tested. 4. Each device must be tagged or labeled so the reports will easily identify the specific location of the device. 5. Operational test of each audible notification zone shall be tested to verify that the fire alarm message corresponds to the correct location. The selective audio messages are in buildings 160, 162, 200, 204, 77, 69 and 8. At minimum, one initiating device shall be verified for each audio zone annually. 6. Check rechargeable cell voltages. 7. Verify that the fiber optic fire alarm network is working correctly in a style 7 configuration. 8. Functional test of all local and remote annunciators. 9. The reports shall indicate the respective duct detector s HVAC unit shut-down status for each duct detector tested. e. Functional test of all audible and visual notification devices to include proper dB and candela levels as per NFPA 72. (Notification devices include Fire Alarm bells, Chimes, Bell Strobes, Horns, Horn Strobes, Speakers and Speaker Strobes). f. Annual functional test required for all alarm output relays-i.e., door controls, fan shutdown, damper control, damper shutdown, elevator recall, security system disconnect, etc. g. Annual test of FACP s for accuracy and lamp display, fuses, etc. h. Functional test for all remote annuciators (All lights, piezio s, displays, and buttons). i. Annually clean all smoke detectors in accordance with NFPA 72 and manufacturers recommendations. Insure that all detectors are remounted properly after cleaning. j. This contract shall include 400 hours of technician maintenance labor hours annually. This time must be documented and approved by Engineering. These maintenance hours shall be included in the cost of this contract and can be requested at any time including after hours and holidays. It is the responsibility of the contractor to maintain written documentation with approval signatures from the VA for all included service hours to track the number of hours used. Any maintenance hours needed over the 400 allotted hours can be billable at the GSA labor rates pending approval of the Safety Office. k. Any parts needed for repairs shall be billed to the VA using GSA pricing. All parts needed for service or maintenance shall be approved by VA Safety Staff prior to being ordered and installed. Warranty replacement parts shall not be billed to the VA. VII. RECORDS AND REPORTS: - The contractor shall maintain management and maintenance records and prepare testing and maintenance reports. All records and copies of reports shall be turned over to the Safety Office. A complete work history file for each fire alarm system for each building shall be maintained by the contractor. a. A daily report shall be submitted to the Safety Office at the end of the work day to indicate what work has been accomplished. b. All inspection and testing reports shall be submitted electronically to the Safety Office monthly no later than the close of business on the last day of the month that the services were performed. c. The report and review of work by the Safety Office representative will be the basis for certifying invoices for payment. A statement that the routine inspection, testing, and maintenance has been performed in accordance with this contract shall be provided. d. Files shall contain a listing of all equipment in each building and structure by nomenclature and manufacturer s model number, as well as any label provided by the VA. All devices on the report must be identifiable by the label on the report. e. The contractor shall provide a separate 3-ring binder for building 162 and 200 annually. The binder shall have dividers for separating the Elements of Performance for The Joint Commission s Environment of Care standard 02.03.05. VIII. SECURITY REQUIREMENTS: a. No later than three days prior to the start of this contract, the contractor shall furnish the Contracting Officers Representative (COR) with a list of employees who will work on-site under this contract with copies of their certifications required in this contract. b. The COR will furnish VA forms (applications for ID card) for each employee working under the contract. The contractor shall complete the applications and deliver them to the VA Police/Security office. The Safety Office will notify the contractor when to pick up the ID badges. The contract company s employees are required to always wear the ID badges while on VA property. The contractor shall report lost ID badges within one working day of the loss to the VA Police Office. c. After this contract or termination of any employees work contract, the contractor shall return the ID badges to the VA Police Office. d. All the contractors employees shall attend the Safety / Infection Control Contractor Training before work is performed. This training must be scheduled with the Safety Office. e. All contractors shall sign the Contractor Log Book at building 37 s reception desk when they arrive and depart each day work is performed. The maintenance hours service reports will be verified with the Contractor Log Book to verify service hours were performed. Service hours will not be counted as hours of work performed unless the contractor has signed for the arrival and departure in the log book. IX. QUALITY ASSURANCE: a. The maintenance of the equipment will be subject to inspection by the Government during and after completion of each task. The contractor shall correct unsatisfactory work promptly. The Government reserves the right to charge the contractor for the cost of Government re-inspection and re-tests because of rejection of unsatisfactory work. b. If during any period when service work is performed on the fire alarm system, the contractor may elect to perform scheduled maintenance at that time. It is the contractor s responsibility to acquire approval by the Safety Office to perform such maintenance. d. Authorization is required to be obtained from the Safety Officer prior to initiating ANY work on ANY fire alarm system at the VA s Mountain Home, TN campus. e. The contractor shall be responsible for the quality of all work performed under this contract. The contractor shall develop and implement a quality control program to assure the requirements of the contract are being fulfilled. X. SAFETY: a. Compliance with Fire Prevention and Safety Regulations The contractor shall, in the performance of this contract, comply with all Federal Fire Prevention Safety laws and regulations, such as VA Safety regulations, policies and procedures, Occupational Safety and Health Administration OSHA, etc., as well as with recognized commercial safety practices and procedures. Prior to the commencement of work, the contractor shall meet with the Safety Officer Representative to discuss and develop mutual understanding relative to administration of the safety program. The contractor shall provide all necessary equipment to perform the work, and at no time will the contractor be permitted to use government equipment. This includes any vertical lift equipment or confined space rescue equipment. b. Death or Injury to Personnel: - In the event of injury to any person in the performance of this contract, the contractor shall furnish a full report of the facts and circumstances to the Safety Office within 24 hours of the incident. In case of emergency, the contractor will call 2911, VA Dispatch Office, from a VA station phone and notify the Safety Office of the location of the accident. c. Barricades or Warnings Signs: - For equipment out of service, the contractor shall be responsible for notifying the Safety Office at least 24 hours in advance of performing periodic maintenance or testing, so that a sign may be placed adjacent to the equipment to warn employees. The contractor shall provide an Out of Service sign at all time while performing maintenance/testing and shall remove the sign after completion of all work. d. Lock Out and Tagging Safety Switch and/or Circuit Breakers: - The contractor shall lock out and tag each safety switch and/or circuit breaker that is turned off citing the following information: - Name of Contractor, date and Reason for shutoff. XI. COMPLIANCE WITH FEDERAL, STATE AND LOCAL LAWS: - The contractor shall be responsible for obtaining any necessary licensure and permits, for complying with any applicable Federal, State and Local laws, codes and regulations relating to the work performed and the protection of workers and property. XII. SITE CONDITIONS: - Asbestos insulation exists in various locations in the buildings located on the VAMC campus. Routine maintenance, testing and inspection work shall be conducted so as not to disturb asbestos. However, if asbestos is suspected and cannot be avoided, notify the Safety Office to determine the course of action. XIII. APPLICABLE PUBLICATIONS: - The publications listed below are applicable to this performance work statement. All materials provided by the contractor shall comply with these specifications and standards. a. National Fire Protection Association (NFPA): NFPA 70 2017 National Electric Code NFPA 72 2016 National Fire Alarm and Signaling Code NFPA 101 2018 Life Safety Code 4. NFPA 70E 2018 Electrical Safety in the Workplace b. Comprehensive Accreditation Manual for Hospitals, The Joint Commission, Environment of Care Management (EC) 2018 Edition, EC.02.03.
