Contracting Activity: Department of Veterans Affairs, VISN 12 Tomah VA Medical Center, 500 E Veterans Street, Tomah, WI 54660 Great Lakes Acquisition Center Network 12 Contract Office 115 South 84th St, 1st Floor, Milwaukee, WI 53214-1476 Purchase Order Request Number 676-19-2-167-0009 has been submitted for this requirement. 2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The Tomah VA Medical Center, Pharmacy Department Services, requires a service agreement for the technical support, maintenance and system upgrades of InstyMeds medication dispensers. This dispenser system is used by both Urgent Care and Pharmacy. The contract will consist of (1) base year plus (4) option years, with an estimated total contract value of $59,400.00. 3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: InstyMeds Corporation is the only vendor capable of providing this service due to Exclusive Licensing and Patent Agreements. InstyMeds provided a proprietary letter on January 11, 2019, confirming that they do not license any other parties to service their equipment. 4. Description of market research conducted and results or statement why it was not conducted: Internet searches revealed zero possible alternatives available in the market to satisfy this requirement. GSA, NAC, SBA, Ability One, google, and UNICOR searches by product name, InstyMeds, and NAICS Code, 541511, were also conducted, resulting in no possible alternatives at this time. Intent to Sole Source was posted on FBO on 02/05/19. 5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief. Since this requirement can be filled only by InstyMeds Corporation, due to licensing and patent agreements, CO does not have a reasonable expectation of receiving two or more quotes from veteran owned small business concerns NOTE: THIS NOTICE WAS NOT POSTED TO FEDBIZOPPS ON THE DATE INDICATED IN THE NOTICE ITSELF (06-MAR-2019); HOWEVER, IT DID APPEAR IN THE FEDBIZOPPS FTP FEED ON THIS DATE.
