This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for Quotations (RFQ) and the solicitation number is 36C24419Q0365. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97 effective January 24, 2018. The Department of Veterans Affairs, Wilmington VA Medical Center, Montchanin Annex; 20 Montchanin. Greenville, DE. 19807 is seeking Carpet Replacement All interested companies shall provide quotations for the following: The anticipated Period of Performance will be being 3/11/2019 following Contract award and includes the following Contract Line Item Numbers (CLIN): **SEE BELOW PRICE/COST SCHEDULE Place of Performance Address: Department of Veterans Affairs Wilmington VA Medical Center Montchanin Annex 20 Montchanin Rd. Greenville, DE 19807 All work shall be completed in accordance with the Statement of Work/Performance Work Statement (SOW/PWS). **The SOW is included as attachment to this solicitation. Offerors are requested to quote Firm Fixed Price for any and all of the line items on the Schedule of Supplies and Services in this solicitation. Service Contract Act Wage Determination Number 2015-4215, Revision Number 7, dated 12/26/2018, applies to this acquisition and will be incorporated into the resulting contract. A copy of the Wage Determination is attached with this solicitation. **The Wage Determination is included as attachment to this solicitation. Any award resulting from this solicitation will be issued on a Standard Form 1449. NOTE: To receive an award resulting from this solicitation, offerors MUST be registered in the System for Award Management (SAM) database IAW FAR 52.212-(k) & VETBIZ.
