- Small Business Participation and Commitment Proposal Format Attachment B SUBCONTRACTOR/TEAMING PARTNER CONSENT LETTER PAST PERFORMANCE DESCRIPTION TEMPLATE/ PAST PERFORMANCE QUESTIONNAIRE/ ATTACHMENT D DD254 Instructions DD254 ATTACHMENT 1a TASK AREA EXPERIENCE Amendment 0005 Amendment 0004 Offerors shall send copies of the Past Performance Questionnaire to vendors. Offerors shall instruct the vendors to return the questionnaire directly to Tiffany Tanner at the Defense Security Cooperation Agency Contracting Office. Past Performance Questionnaires may be sent via email to dsca.ncr.dbo.mbx.cons@mail.mil and cc to tiffany.n.tanner4.civ@mail.mil. OR mailed or hand delivered to Defense Security Cooperation Agency 201 12th Street South, Arlington, VA 22202. Questionnaires are due 15 November 2018 @ 12:00 pm, EST. The purpose of this amendment is to make changes to Section "L" and Section "M". Also to update the required response date/time has changed from 13-Nov-2018 12:00 PM to 15-Nov-2018 12:00 PM. Amendment 0004 and 0005 are now uploaded. There is not an Amendment 0006 at this time. Amendment 0006 will be post in the near future with answers to industry questions. Thank you for your patience
