Hello.   The following is a Request for Information.   We hope your firm decides to participate. -Begin Request for Information- REQUEST FOR INFORMATION / SOURCES SOUGHT for Hospital Examination Privacy Curtains, Brand Name Only, Simple 66 DEPARTMENT OF VETERANS AFFAIRS, VISN 23 Source Sought:   The VA is seeking business sources qualified to provide the items listed below as Brand Name.   This is NOT a solicitation. We hope to solicit this later as an SDVOSB set-aside. INTERESTED SOURCES SHALL RESPOND BY: Please submit response no later than 2:00pm CST Thursday, November 8, 2018 All responses and questions must be emailed to andrew.hinton3@va.gov and must include in the subject line:   Sources Sought, Hospital Examination Privacy Curtains. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1.       Company name, address, phone number, primary contact(s), e-mail address. 2.                       State if your firm distributes the Brand Name items. 3.                       DUNS number. 4.                       NAICS (North American Industry Classification System) code that your firm uses for hospital supplies. 5. State your business size and category (example: VOSB, SDVOSB). We intend to solicit this as an SDVOSB set-aside. We will verify business category in VetBiz/ VIP prior to RFQ and prior to award. 6.                       State if your firm has required items on Schedule (FSS, GSA, other ).                               REQUIRED ITEM and Salient Characteristics: Primary Care Snap Panel Curtains, Brand-Name Only: Brand Name: Simple 66 Item: Snap Panels Quantity Required: 117 Each Size each: 66 Wide x 66 Long Color: White Pattern: Plain White Optional Line Items Warranty Information 1 Year Warranty or Manufacturer s Warranty Whichever is greater. Delivery Requirements: All products should be ordered to arrive on or about 1-2-2018. Please include any shipping costs that are not included in the cost of the line items.
