- Request for Quotation Instructions The Mission and Installation Contracting Command at Joint Base Lewis McChord, WA, have a requirement for an ordained Roman Catholic Priest, to perform Pastoral Services at the Madigan Army Medical Center. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. RFQ Content: 1.Quoters shall complete and submit the provided Price Schedule, in its entirety. 2.Quoters shall complete and submit paragraph (b) (2) of the FAR Provision 52.212-3 if the annual representations and certifications have been completed electronically. If a quoter has not completed the annual representations and certifications electronically at the SAM website, the quoter shall manually complete only paragraphs (c) through (u) of this provision. 3.Provide the official DD Form 2088, Statement of Ecclesiastical Endorsement, issued by the Archdiocese of Military Service 4.Provide the official transcripts showing proof of a minimum of (2) two units of clinical pastoral education.
