- RFQ Amendment 4 Pricing and Delivery Templet RFQ Amendment 4 Questions and Answers RFQ Amendment 4 SF-18, SOW Terms and Conditions Amendment 0004 BPA and 1st Order for SBA-ODA Logo Polo Shirts The purpose of amendment 0004 is to post questions and answers, a revised RFQ SOW and Terms and Conditions and to post a revised sample templet for pricing the BPA and the first task order and to give a sample of a delivery schedule. This amendment hereby replaces the original RFQ SOW and Terms and Conditions and the prior amendments. Quotes are due via email no later than September 18, 2018 at 1:00 pm ET. Sample shirts is due no later than September 18, 2018. This is a total Small Business set aside.
