- Attachment 6 - Wage Determination Attachment 5 - Recent and Relevant Experience reference Sheet Attachment 4 - Instructions for Submissions of Quotes Attachment 3 - Bid Schedule Attachment 2 - Clauses and Provisions Attachment 1 - Statement of Work_Fire Extinguisher BPA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Joint Base Lewis-McChord, WA, has a requirement for fire extinguisher services and intends to award Blanket Purchase Agreement(s) (BPA) for a 1-year term, and issue subsequent call orders thereunder which will be invoiced directly to the calling officer and paid via Government Purchase Card (GPC). This solicitation is issued as a Request for Quotes using Low Priced- Technically Acceptable selection procedure. One (1) to three (3) Blanket Purchase Agreement(s) will be awarded as a result. Offerors shall consider the information included in all solicitation attachments, and shall submit responses in accordance with these instructions. To be considered responsive, offerors shall submit completed Schedule of Services-Prices, Recent and Relevant Experience Reference Sheet, and certificates or evidence showing ability to meet National Fire Protection Association (NFPA) 10 requirements as stated in the Statement Of Work (SOW). Failure to provide these documents by the date and time set for responses may result in the quote being considered non-responsive and ineligible for further consideration. Offerors shall submit responses via email to both Robert Blair, Contract Specialist, and Jose Diaz, contracting officer, at the following addresses: robert.j.blair.civ@mail.mil and jose.f.diaz1.civ@mail.mil, no later than 12:00 noon Pacific Time on the due date set for responses. Questions regarding this solicitation shall be directed to the contract specialist and contracting officer via email to the addresses above, preferably no later than three (3) business days prior to the due date for received later may not provide sufficient time for adequate response, and the government reserves the right to proceed to close the solicitation on schedule regardless.
